Os autores apresentam uma sistematização das lesões que podem ser encontradas na moléstia de Recklinghausen. Foram observados 4 casos de forma nervosa central da moléstia. Em todos existiam tumores bilaterais nos ângulos pontocerebelares, constituindo uma das manifestações mais importantes da doença, sendo mesmo a única em um dos casos (forma central pura). Nos demais, portadores de formas mistas, além de outras lesões centrais (neurofibromas, meningeomas, ninhos de células atípicas), havia lesões periféricas discretas. Tumores intrarraquidianos originados nas bainhas radiculares foram os causadores da sintomatologia inicial em 3 pacientes. Em dois casos em que o córtex foi examinado histológicamente, foram encontrados ninhos de células gliais atípicas, característicos da forma central da neurofibromatose. Duas lesões relativamente raras foram encontradas no segundo caso: angioma cortical e neurite intersticial hipertrófica do plexo braquial. Em pacientes portadores de neurofibromatose periférica ou raquídea, como também em seus familiares, devem ser sempre procuradas lesões centrais, principalmente tumores do nervo acústico. O exame neuro-radiológico de escolha para confirmar o diagnóstico dêstes tumores é a iodoventriculografia.

Abstract in English:

The authors present a systematization of the lesions seen in Recklinghausen's disease. Four cases of the nervous central type of the disease were studied. In all the cases there were bilateral pontine-cerebellar tumors, one of the most important findings for diagnostic purposes, which was the only manifestation of the disease in one of them (pure central form). In the others, with mixed types, besides various central lesions (neurofibromas, meningeomas) there were mild peripheral lesions. Intravertebral tumors orinating from radicular sheaths were the cause of the initial symptoms in three patients. In the two cases where the cerebral cortex was histologically studied, groups of atypical glia cells characteristic of the central type of neurofibromatosis were found. Two relatively rare lesions were found in the second case : cortical angioma and hypertrophic intersticial neuritis of the brachial plexus. In patients with peripheral or intravertebral neurofibromatosis, as well as in their relatives, central lesions must be sought for, especially tumors of the acoustic nerve. The neurological diagnostic procedure of choice in such tumors is the contrasted ventriculography with Lipiodol.

Após considerações preliminares sôbre as moléstias decorrentes de alterações do metabolismo das porfirinas e que se traduzem clinicamente por alterações cutâneas, digestivas, neurológicas e psíquicas, é apresentado o caso de uma paciente portadora de forma aguda de porfiria. Trata-se de paciente com 29 anos de idade, com antecedentes de dores nos membros e cólicas abdominais, que apresentou bruscamente quadro de paralisia motora dos quatro membros e de alguns nervos cranianos. O teste de Watson e Schwartz revelou a presença de porfobilinogênio na urina, o que firmou o diagnóstico. Após um ano de internação, durante o qual a moléstia evoluiu por surtos, a paciente faleceu, tendo o exame anátomo-patológico do sistema nervoso revelado alterações degenerativas das células motoras das pontas anteriores da medula e de núcleos motores do tronco encefálico.

Abstract in English:

After general considerations on diseases with alterations of the porphyrin metabolism a case of acute form of porphyria is reported. The patient, 29 years old, female, with pains in the arms and legs and abdominal colics, suddenly, presented motor paralysis of arms, legs and some of the cranial nerves. Confirming the diagnosis of porphyria, the Watson and Schwartz test demonstrated presence of porphobilinogen in the urine. After being interned in the hospital for a whole year, during which the disease developed in surges, the patient died. The anatomo-pathological study of the nervous system showed degenerative alterations of motor cells of the anterior horns and mesencephalic motor nuclei.

Apresentação de um caso de forma paraplégica de tétano, com ponto de partida no pé, verificada em paciente portador de lesão medular pregressa (inflamatória? desmielinizante?). Os autores levantam a hipótese de que seu caso seria a reprodução, na patologia humana, das conclusões experimentais de Meyer, Ranson e outros, que conseguiram limitar o tétano aos membros posteriores de animais de laboratório quando, previamente à injeção de toxina na pata posterior, seccionavam a medula espinal.

Abstract in English:

Report of a paraplegic form of tetanus, after a foot's wound, in a patient with a pregress spinal cord disease (inflammatory, demyclinating?). The authors suggest that this case would be the reproduction in human pathology of the experimental works of Meyer, Ranson and others, demonstrating the possibility to limit the process to the posterior train of animals when, previously to the toxin injection in one of the posterior limbs, the spinal cord was transected.

Os caracteres gerais do método eletroforético são apresentados de modo sumário, especialmente aquêles referentes à eletroforese em papel, no sentido de familiarizar o neurologista quanto aos seus aspectos principais e sua utilização no estudo das frações proteicas do liqüido cefalorraquidiano cujos aspectos particulares são discutidos.

The main aspects of electrophoresis are summarised, specially those concerning paper electrophoresis, as a practical guide for neurologists, in its appliance to the study of cerebrospinal fluid protein fractions.
