RESUMO Objetivo Este estudo teve o objetivo de investigar a estabilidade das medidas de frequência fundamental da voz, comparando amostras de fala habitual e de fala em situação de disfarce. Método Trata-se de estudo observacional, analítico e transversal. Foi analisado um banco de vozes de 40 pessoas de ambos os sexos (20 homens e 20 mulheres) e faixa etária entre 19 e 58 anos, correspondentes à leitura de trecho em fala habitual e leitura do mesmo trecho em fala em situação de disfarce. Os tipos de disfarces foram analisados por três juízas especialistas em voz. Além disso, foi utilizado software de análise acústica PRAAT® para a extração das medidas acústicas de f0 média, f0 mediana, f0 mínima, f0 máxima e f0 de base (fb) da leitura na íntegra, em ambas as situações, por meio de procedimentos pré-determinados. Os dados foram tabulados e analisados estatisticamente. Resultados Os valores das medidas de fb, f0 máxima e f0 mínima foram semelhantes nas situações de fala habitual e disfarce, para locutores de ambos os sexos. Já os valores das medidas de f0 média e f0 mediana tiveram diferença nas duas situações. Conclusão De modo geral, as medidas de fb, f0 mínima e f0 máxima mostraram maior estabilidade frente à situação de disfarce, o que pode sugerir utilidade futura em comparações intraindividuais. Os variados tipos de ajustes feitos pelos locutores para a realização dos disfarces não alteraram de modo significativo as medidas de fb, f0 máxima e f0 mínima quando as situações de fala habitual e disfarçada foram comparadas.

ABSTRACT Purpose This study aimed to investigate the stability of measures of fundamental frequency of voice by comparing samples of habitual and disguised speech. Methods This observational, analytical, cross-sectional study analyzed a voice bank of 40 people of both sexes (20 men and 20 women), aged 19 to 58 years. They read a passage in both habitual and disguised speech. The types of disguises were analyzed by three expert voice judges. The study used the PRAAT® acoustic analysis software to extract the mean f0, median f0, minimum f0, maximum f0, and base f0 (fb) from the complete reading in both situations, using pre-determined procedures. The data were tabulated and statistically analyzed. Results The fb, maximum f0, and minimum f0 values were similar in habitual and disguised speech among speakers of both sexes, whereas the mean f0 and median f0 differed between the two situations. Conclusion In general, the measures of fb, minimum f0, and maximum f0 were more stable in disguised speech, which may suggest future usefulness in intraindividual comparisons. The speakers’ various types of adjustments to disguise their voices did not significantly alter the measures of fb, maximum f0, and minimum f0 in the comparison between habitual and disguised speech.

Adaptação de aparelho de amplificação sonora individual: análise do impacto no sistema auditivo durante o primeiro ano de uso em perdas auditivas moderada e severa Calderaro, Victor Goiris Amaral, Maria Stella Arantes do Massuda, Eduardo Tanaka Reis, Ana Cláudia Mirândola Barbosa

RESUMO Objetivo Investigar o comportamento do sistema auditivo em indivíduos que fazem uso de aparelhos de amplificação sonora individual, ao longo do primeiro ano de uso. Método Participaram do estudo 24 indivíduos, de ambos os sexos, com perda auditiva sensorioneural moderada ou severa. A primeira etapa de coleta ocorreu antes da adaptação dos aparelhos de amplificação sonora individual, mediante a aplicação do exame eletrofisiológico P300 e do teste Listas de Sentenças em Português. A segunda e terceira etapas ocorreram 6 e 12 meses após o início da reabilitação, sendo aplicados os dois testes mencionados e o questionárioHearing Handicap Inventory for AdultsouHearing Handicap Inventory for The Elderly. Resultados Não houve diferença na latência e amplitude do P300 entre as etapas, tanto quando realizado com tone burst, como para estímulo de fala. Os limiares auditivos pós adaptação permitiram acessibilidade aos sons da fala, diferindo, portanto, a etapa 1 das etapas 2 e 3, sem diferença entre as duas últimas. Houve evidência de melhora na percepção da fala ao longo das três fases, principalmente nos indivíduos com perda auditiva de grau moderado. O questionário de autoavaliação mostrou que o uso de aparelhos de amplificação sonora individual reduziu a restrição de participação percebida em contextos emocionais e sociais. Conclusão Embora não haja alterações nas medidas eletrofisiológicas ao longo do primeiro ano de uso, observou-se melhora na percepção da fala (reconhecimento). Além disso, os usuários relatam uma melhora na qualidade de vida, evidenciando os benefícios do uso de aparelhos de amplificação sonora individual.

ABSTRACT Purpose To investigate the behavior of the auditory system in people’s first year of hearing aid use. Methods The study included 24 individuals of both sexes with moderate and severe sensorineural hearing loss. The first data collection stage occurred before fitting the hearing aids, through the P300 electrophysiological examination and the Portuguese Sentence List test. The second and third stages occurred 6 and 12 months after starting rehabilitation, applying these two tests and the Hearing Handicap Inventory for Adults or Hearing Handicap Inventory for the Elderly. Results The P300 latency and amplitude did not differ between the stages with either tone-burst or speech stimulus. The post-fitting hearing thresholds allowed accessibility to speech sounds, therefore differing between stage 1 and stages 2 and 3, with no difference between the last two. There was evidence of improved speech perception throughout the three phases, particularly in individuals with moderate hearing loss. The self-assessment questionnaire showed that the use of hearing aids reduced perceived participation restrictions in emotional and social contexts. Conclusion Despite the lack of changes in electrophysiological measurements during the first year of use, speech perception (recognition) improved. Users also reported an improvement in quality of life, highlighting the benefits of using hearing aids.

RESUMO Objetivo Verificar o efeito do Treinamento Auditivo Musical (TAM) na neuroplasticidade do sistema auditivo e na percepção do zumbido em adultos. Método Ensaio clínico randomizado duplo-cego, aprovado pelo Comitê de Ética em Pesquisa. Foram incluídos vinte adultos com queixa de zumbido há pelo menos seis meses, intensidade maior que quatro pontos na Escala Visual Analógica (EVA), limiares auditivos normais bilateralmente e sincronia neural preservada em tronco encefálico. Os participantes foram distribuídos aleatoriamente em dois grupos: Controle (GC, n=10), submetido à estimulação passiva com material audiovisual sem tarefas, e Estudo (GE, n=10), submetido ao TAM. Ambos realizaram oito sessões, duas vezes por semana, ao longo de quatro semanas. As avaliações compreenderam Potenciais Evocados Auditivos de Longa Latência (PEALL) verbais, para investigar neuroplasticidade, além da EVA (volume e incômodo) e do Tinnitus Handicap Inventory (THI), para mensurar a percepção do sintoma. Resultados Ambos os grupos apresentaram melhora na percepção do zumbido após a intervenção. Contudo, a análise diferenciada mostrou que o GC apresentou mudanças restritas, com melhora apenas na duração do componente P3 dos PEALL. Já o GE apresentou alterações mais amplas, incluindo mudanças significativas na amplitude e duração do P3, indicando maior recrutamento neural associado ao processamento auditivo. Além disso, apenas o GE demonstrou reduções consistentes no volume e no incômodo do zumbido, bem como no impacto sobre a qualidade de vida, refletido nas pontuações do THI. Conclusão Após quatro semanas, o TAM mostrou-se superior à estimulação passiva, promovendo alterações positivas na neuroplasticidade auditiva, diminuição mais expressiva da intensidade e do incômodo do zumbido e melhora significativa na qualidade de vida dos participantes.

ABSTRACT Purpose To evaluate the effect of Musical Auditory Training (MAT) on auditory system neuroplasticity and on tinnitus perception in adults. Methods A randomized double-blind clinical trial, approved by the Research Ethics Committee. Twenty adults with tinnitus complaints for at least six months, intensity greater than four points on the Visual Analog Scale (VAS), normal bilateral hearing thresholds, and preserved neural synchrony at the brainstem level were included. Participants were randomly assigned to two groups: Control Group (CG, n=10), submitted to passive stimulation with audiovisual material without tasks, and Study Group (SG, n=10), submitted to MAT. Both groups underwent eight sessions, twice a week, over a four-week period. Assessments included verbal Long-Latency Auditory Evoked Potentials (LLAEP), to investigate neuroplasticity, as well as the VAS (volume and discomfort) and the Tinnitus Handicap Inventory (THI), to measure symptom perception. Results Both groups showed improvement in tinnitus perception after the intervention. However, a differentiated analysis revealed that the CG presented restricted changes, with improvement only in the duration of the P3 component of the LLAEP. In contrast, the SG showed broader changes, including significant alterations in both amplitude and duration of P3, suggesting greater neural recruitment associated with auditory processing. In addition, only the SG demonstrated consistent reductions in tinnitus volume and discomfort, as well as in the impact on quality of life, as reflected in THI scores. Conclusion After four weeks, MAT proved to be superior to passive stimulation, promoting positive changes in auditory neuroplasticity, a greater reduction in tinnitus intensity and annoyance, and a significant improvement in participants’ quality of life.

Medidas acústicas espectrais, cepstrais e de curto prazo em mulheres com e sem alterações laríngeas e vocais Gualberto, Amanda Mariane Paulino Penido, Fabiana Andrade Gama, Ana Cristina Côrtes

RESUMO Objetivo Comparar medidas acústicas da voz de mulheres com e sem alterações laríngeas e vocais. Métodos Estudo observacional analítico transversal com 69 professoras, divididas em quatro grupos: 21 com alterações laríngeas (GCA), 48 sem alterações (GSA), 45 com desvios vocais (GCD) e 24 sem desvios (GSD). Os grupos foram definidos pela avaliação otorrinolaringológica (videolaringoscopia ou videolaringoestroboscopia) e pela avaliação perceptivo-auditiva do grau de disfonia. Foram excluídas participantes fumantes, grávidas, em período menstrual, com alergias ou gripes. Amostras de voz e fala (3 segundos centrais da vogal [a] e contagem de 1 a 11) foram gravadas e editadas no Praat. As medidas acústicas Jitter, Shimmer, Harmonic Noise Ratio (HNR), Glottal to Noise Excitation (GNE), Smoothed Cepstral Peak Prominence (CPPS), Spectral tilt, High Frequency Noise (HF noise) e a diferença entre o primeiro e segundo harmônico (H1-H2) foram extraídas no VOXplot. A análise estatística foi realizada por meio do teste T, Mann-Whitney e Hedges’ g. Resultados Mulheres com alterações laríngeas e vocais apresentaram valores menores de GNE, HNR, CPPS, H1-H2 e HF noise e valores maiores de Jitter, Shimmer e Spectral tilt. O CPPS, o Spectral tilt (vogal) e o H1–H2 (vogal e fala) não diferenciaram os grupos com e sem alterações laríngeas. O CPPS (vogal) e o Spectral tilt (vogal e fala) também não diferenciaram os grupos com e sem desvio vocal. Conclusão As medidas acústicas responderam de forma semelhante à presença de alterações laríngeas e vocais, reforçando a influência conjunta de fatores estruturais e funcionais na qualidade do sinal vocal.

ABSTRACT Purpose To compare acoustic voice measures of women with and without laryngeal and vocal alterations. Methods Cross-sectional, analytical, observational study with 69 female teachers, divided into four groups: 21 with laryngeal alterations (LAG), 48 without alterations (NAG), 45 with vocal deviations (VDG), and 24 without deviations (NDG). The groups were defined by otolaryngological evaluation (videolaryngoscopy or videolaryngostroboscopy) and by auditory-perceptual evaluation of the grade of hoarseness. Participants who smoked, were pregnant, menstruating, or had allergies or the flu were excluded. Voice and speech samples (3 central seconds of the vowel [a] and counting from 1 to 11) were recorded and edited using Praat. The acoustic measures jitter, shimmer, harmonic-to-noise ratio (HNR), glottal-to-noise excitation (GNE), smoothed cepstral peak prominence (CPPS), spectral tilt, high-frequency noise (HF noise), and the difference between the first and second harmonics (H1-H2) were extracted from the VOXplot. Statistical analysis was performed using the t-test, Mann-Whitney test, and Hedges' g test. Results Women with laryngeal and vocal alterations had lower GNE, HNR, CPPS, H1-H2, and HF noise values and higher jitter, shimmer, and spectral tilt values. CPPS, spectral tilt (vowel), and H1-H2 (vowel and speech) were not different between the groups with and without laryngeal alterations. CPPS (vowel) and spectral tilt (vowel and speech) were likewise not different between the groups with and without vocal deviation. Conclusion The acoustic measures responded similarly to the presence of laryngeal and vocal alterations, reinforcing the combined influence of structural and functional factors on the quality of the vocal signal.

Sleep quality and risk of oropharyngeal dysphagia in older people in intensive care units Lira, Isabela Cristina Monteiro de Martins, Natália de Castro e Silva Oliveira Filho, Luciano Pedrosa de Silva, Bianca Emmanuely Clemente da Studart-Pereira, Luciana Moraes

ABSTRACT Purpose To associate the risk of oropharyngeal dysphagia with poor sleep quality in older adults in intensive care units. Methods Analytical, observational, cross-sectional study carried out in intensive care units with purposive sampling. A screening form was completed daily for patients admitted during the research period, totaling 221 participants. The inclusion criteria were older individuals (> 60 years old) with preserved cognitive status, communication skills, and physical and emotional stability; based on these criteria, the sample comprised 66 participants. The study used the Richards-Campbell Sleep Questionnaire to assess sleep quality, item 4 of the Sleep in ICU Questionnaire to assess daytime sleepiness, and the Oropharyngeal Dysphagia Screening in Hospitalized Older Adults to assess the risk of dysphagia. Results There was a predominance of females (65.2%) and of the age group of 60-70 years (66.7%). Of these, 30.3% were hospitalized due to respiratory problems. Most of the sample (62.1%) reported good sleep quality, while 37.9% had poor sleep. Sleep depth and time to sleep lowest scores, reflecting increased sleep latency and reduced sleep depth. Also, 16.7% scored at risk for oropharyngeal dysphagia during hospitalization, with choking (24.2%) and respiratory incoordination (22.7%) being the most prevalent risk signs in the study population. The risk of oropharyngeal dysphagia was not significantly associated with poor sleep quality (p > 0.05). Conclusion Sleep quality was not associated with the risk of dysphagia in older adults admitted to intensive care units.

Effectiveness of a therapeutic program with chin tuck against resistance exercises compared to the chin-down maneuver in Parkinson's disease: a randomized controlled pilot trial Araújo, Ramon Cipriano Pacheco de Taveira, Karinna Veríssimo Meira Godoy, Juliana Fernandes Magalhães, Hipólito

ABSTRACT Purpose Investigate the effectiveness of a therapeutic program with chin tuck against resistance (CTAR) exercises, compared to the chin-down (CD) maneuver, on signs suggestive of dysphagia, maximum isometric tongue pressure, and therapeutic efficacy in individuals with Parkinson’s disease (PD). Methods 26 individuals with primary PD were randomly assigned to two intervention groups for four weeks (one session per week). Group A performed the CTAR exercises (n = 13), and Group B performed the CD postural maneuver with swallowing training with different food consistencies (n = 13). The outcomes assessed were signs suggestive of dysphagia using the Northwestern Dysphagia Patient Check Sheet (NDPCS), maximum isometric tongue pressure (MIP), the Eating Assessment Tool (EAT-10) to monitor therapeutic efficacy, and peak cough flow, before and after the intervention period. Results The CTAR group showed a significant improvement in items related to the NDPCS, MIP, and EAT-10 compared to the CD group (p < 0.05). There was no significant difference in peak cough flow in either group. Conclusion PD patients who underwent an intervention program with CTAR exercises showed an improvement in signs suggestive of dysphagia and an increase in maximum isometric tongue pressure, compared to those who underwent the therapeutic program with the CD postural maneuver.

Aspectos comportamentais e funcionais em escolares do Ensino Fundamental de escolas públicas e privadas Almeida, Maria Júlia Amaral Abranches de Fernandes, Graziela Nunes Alfenas Lemos, Stela Maris Aguiar

RESUMO Objetivo Investigar a relação entre aspectos comportamentais (dificuldades e capacidades) e a funcionalidade de estudantes do Ensino Fundamental II, considerando variáveis como ano escolar, sexo, idade, classe econômica e tipo de escola. Método Estudo observacional e transversal com 157 escolares de 11 a 14 anos, matriculados em escolas públicas e privadas. Foram utilizados os instrumentos Caracterização dos Participantes, Critério de Classificação Econômica Brasil (CCEB), Strengths and Difficulties Questionnaire (SDQ-Por) e a Classificação Internacional de Funcionalidade, Incapacidade e Saúde (CIF), aplicados via Google Forms. A análise foi realizada com o software SPSS (versão 25.0). Resultados A maioria dos participantes apresentou classificação normal nas escalas do SDQ e foi avaliada como “sem dificuldades” nas categorias da CIF. Houve associações estatisticamente significativas entre sintomas emocionais do SDQ e os códigos da CIF d240, d710 e d750, bem como entre problemas de conduta e os códigos CIF d240, d710, d720 e d750. Sintomas de hiperatividade apresentaram associação com sexo e os códigos CIF d240, d710 e d720, enquanto problemas de relacionamento com colegas se associaram aos códigos d750 e d710. Comportamento pró-social foi associado ao sexo e a códigos CIF d240, d710 e d750. O escore total do SDQ apresentou associação com os códigos CIF. Conclusão Destaca-se a importância de compreender as especificidades comportamentais e funcionais dos adolescentes para promover uma melhor qualidade de vida socioemocional nessa faixa etária.

ABSTRACT Purpose To investigate the relationship between behavioral aspects (difficulties and abilities) and the functioning of middle school students, considering variables such as grade in school, sex, age, economic class, and type of school. Methods Observational, cross-sectional study with 157 students aged 11 to 14 years, enrolled in public and private schools. The instruments used were participant characterization, Brazilian Economic Classification Criteria (CCEB), Strengths and Difficulties Questionnaire (SDQ-Por), and the International Classification of Functioning, Disability, and Health (ICF), applied via Google Forms. The analysis was performed using SPSS software (version 25.0). Results Most participants had a normal classification on the SDQ scales and were evaluated as "no difficulties" in the ICF categories. There were statistically significant associations between SDQ emotional symptoms and ICF codes d240, d710, and d750, as well as between conduct problems and ICF codes d240, d710, d720, and d750. Symptoms of hyperactivity were associated with sex and ICF codes d240, d710, and d720, while peer relationship problems were associated with codes d750 and d710. Prosocial behavior was associated with sex and ICF codes d240, d710, and d750. The total SDQ score was associated with the ICF codes. Conclusion The study highlights the importance of understanding the behavioral and functional specificities of adolescents to promote a better socio-emotional quality of life among them.

Postura habitual dos lábios e da língua em lactentes com Trissomia do 21: caracterização e fatores associados Godinho, Letícia Pereira Quintaes Campos, Larissa Melgaço Motta, Andréa Rodrigues Furlan, Renata Maria Moreira Moraes

RESUMO Objetivo Caracterizar a postura habitual de lábios e de língua em lactentes com trissomia do 21 e verificar sua associação com idade, sexo e características clínicas. Método Trata-se de um estudo observacional, transversal e analítico. A amostra foi composta de 61 lactentes com trissomia do 21, com média de idade de 7,4±6,2 meses. A postura habitual de lábios e de língua foi obtida por meio da análise de vídeos da face dos lactentes. Dados referentes a idade, sexo e condições clínicas foram coletados nos prontuários dos participantes. As variáveis resposta do estudo referem-se às posturas habituais de língua e de lábios predominantemente adotadas pelos lactentes. Constituíram variáveis explicativas: idade, sexo, prematuridade, peso ao nascimento, presença de cardiopatia, hipotireoidismo e doença pulmonar, amamentação e uso de mamadeira e chupeta. Os dados foram analisados por meio de estatística descritiva e teste Qui-quadrado de Pearson para associação entre as variáveis, com nível de significância de 5%. O coeficiente Kappa foi utilizado para obter as concordâncias intra e interavaliador em 20% dos vídeos, sendo classificadas como excelente e substancial, respectivamente. Resultados As posturas adotadas por mais tempo foram de lábios semiabertos (48,2%) e de língua dentro da cavidade oral (32,4%). Houve associação da postura de lábios com a idade na data da avaliação e com o peso ao nascimento, e entre a postura de língua e a amamentação. Conclusão Menor idade cronológica no momento da avaliação e maior peso ao nascer estiveram associados à melhor postura de lábios, enquanto a presença da amamentação esteve associada à melhor postura de língua.

ABSTRACT Purpose To characterize the habitual lip and tongue posture in infants with trisomy 21 and to verify its association with age, sex, and clinical characteristics. Methods This is an observational, cross-sectional, analytical study with a sample of 61 infants with trisomy 21, with a mean age of 7.4 ± 6.2 months. The habitual lip and tongue posture was obtained through the analysis of videos of the infants' faces. Data regarding age, sex, and clinical conditions were collected from the participants' medical records. The study response variables were the habitual tongue and lip postures predominantly adopted by the infants. Explanatory variables included age, sex, prematurity, birth weight, heart disease, hypothyroidism, lung disease, breastfeeding, and use of bottles and pacifiers. Data were analyzed using descriptive statistics and Pearson's chi-square test for association between variables, with a 5% significance level. The study used 20% of the videos to verify the kappa coefficient and obtain intrarater and interrater agreement, classified as excellent and substantial, respectively. Results The longest postures were parted lips (48.2%) and tongue inside the oral cavity (32.4%). Lip posture was associated with age at the time of assessment and birth weight, and tongue posture was associated with breastfeeding. Conclusion Younger chronological age at the time of assessment and higher birth weight were associated with better lip posture, while breastfeeding was associated with better tongue posture.

Validação e normatização da Escala de Autopercepção de Habilidades do Processamento Auditivo Central (EAPAC) - expandida Jesus, Luciana Cássia de Alves, Luciana Mendonça Resende, Luciana Macedo de

RESUMO Objetivo validar e normatizar a aplicação on-line da Escala de Autopercepção de Habilidades do Processamento Auditivo Central (EAPAC) – expandida para adultos brasileiros. Método A Escala de Autopercepção de Habilidades do Processamento Auditivo Central foi expandida e respondida por 1274 indivíduos brasileiros com idade entre 18 e 59 anos. Desses, 116 sujeitos foram submetidos à avaliação neuropsicológica e auditiva e 63 foram selecionados para responder à escala em um segundo momento. Foram excluídos aqueles com perda auditiva de qualquer tipo e grau, histórico de terapia fonoaudiológica para o transtorno do processamento auditivo central e alteração psiquiátrica e/ou neurológica. Foi realizada a análise descritiva das variáveis, a análise fatorial exploratória e a curva ROC, para validação e normatização. O teste de Wilcoxon foi utilizado para comparar as respostas dos sujeitos que responderam à escala em dois momentos. Resultados Após a análise fatorial exploratória, cinco questões foram excluídas, pois não correlacionaram com os domínios da escala. As curvas ROC, construídas considerando a presença do transtorno do processamento auditivo e o resultado por processos auditivos, não apontaram valores significativos, sendo assim, a escala apresentou valores frágeis de sensibilidade e especificidade. Não houve diferença significativa entre as respostas dos sujeitos, na escala, em dois momentos diferentes. Conclusão A escala expandida não apresentou padrões psicométricos adequados para identificação do risco para o transtorno do processamento auditivo central, mas mostrou-se útil para investigar as queixas auditivas em tarefas do cotidiano, e, assim, auxiliar na seleção de estratégias para minimizar o impacto negativo das dificuldades referidas.

ABSTRACT Purpose To validate and standardize the online application of the Scale of Self-Perception of Central Auditory Processing Skills (EAPAC) – expanded for Brazilian adults. Methods The EAPAC was expanded and answered by 1,274 Brazilian individuals aged 18 to 59 years. Of these, 116 subjects underwent neuropsychological and auditory evaluation, and 63 were selected to answer the scale at a second time point. Those with any type and degree of hearing loss, history of speech-language-hearing therapy for central auditory processing disorder, and psychiatric and/or neurological alteration were excluded. Descriptive analysis of the variables, exploratory factor analysis, and ROC curve were performed for validation and standardization. The Wilcoxon test was used to compare the responses of the subjects who answered the scale at two time points. Results After exploratory factor analysis, five questions were excluded, as they did not correlate with the scale domains. The ROC curves, constructed considering the presence of auditory processing disorder and the result by auditory processes, did not indicate significant values. Thus, the scale presented fragile sensitivity and specificity values. There was no significant difference in the subjects’ responses to the scale between the two time points. Conclusion The expanded scale did not present adequate psychometric standards for identifying the risk for central auditory processing disorder, but it proved useful for investigating auditory complaints in everyday tasks, thus helping to select strategies to minimize the negative impact of the difficulties mentioned.

Efeitos da estimulação elétrica nervosa transcutânea e da terapia manual laríngea na disfonia por tensão muscular: ensaio clínico randomizado Lemos, Isadora de Oliveira Silvério, Kelly Cristina Alves Cassol, Mauriceia

RESUMO Objetivo Comparar os efeitos imediatos da Estimulação Elétrica Nervosa Transcutânea (TENS), da Terapia Manual Laríngea (TML) e da aplicação combinada de ambas em mulheres com Disfonia por Tensão Muscular (DTM), considerando sintomas vocais, qualidade vocal, medidas aerodinâmicas e níveis de sensação de tensão muscular à palpação. Método Trata-se de um ensaio clínico randomizado. Foram incluídas mulheres com diagnóstico fonoaudiológico de Disfonia por Tensão Muscular, alocadas em três grupos de intervenção: TENS, TML e TENS+TML. As participantes foram avaliadas quanto aos sintomas vocais, intensidade da dor musculoesquelética, avaliação perceptivo-auditiva da qualidade vocal pela escala GRBAS, tempos máximos de fonação (TMF), relação S/Z e níveis de sensação de tensão muscular à palpação com o protocolo Laryngeal Palpatory Scale (LPS). Resultados Observou-se redução significativa dos sintomas vocais, incluindo fadiga vocal, e de dor musculoesquelética após as intervenções, bem como aumento dos TMF em todos os grupos. No grupo TENS, houve redução da soprosidade vocal; no grupo TML, observou-se redução significativa do grau geral da alteração vocal; e no grupo TENS+TML, uma redução do grau geral de alteração e da tensão vocal. O LPS demonstrou diminuição significativa da sensação de tensão muscular à fonação no grupo TENS+TML. Conclusão Todas as intervenções apresentaram efeitos positivos, com redução significativa dos sintomas vocais e da dor musculoesquelética, além do aumento dos tempos máximos de fonação. As técnicas combinadas foram capazes de proporcionar melhores ajustes vocais e diminuir a sensação de tensão muscular à palpação, evidenciando que ambas as abordagens foram eficazes em promover benefícios vocais e musculoesqueléticos.

ABSTRACT Purpose To compare the immediate effects of Transcutaneous Electrical Nerve Stimulation (TENS), Laryngeal Manual Therapy (LMT), and the combined application of both in women with Muscle Tension Dysphonia (MTD), considering vocal symptoms, voice quality, aerodynamic measures, and levels of perceived muscle tension on palpation. Methods This was a randomized clinical trial including women with a speech-language diagnosis of MTD, allocated into three intervention groups: TENS, LMT, and TENS+LMT. Participants were assessed for vocal symptoms, musculoskeletal pain intensity, perceptual-auditory evaluation of voice quality using the GRBAS scale, maximum phonation times (MPT), S/Z ratio, and levels of perceived muscle tension on palpation using the Laryngeal Palpatory Scale (LPS). Results Significant reductions in vocal symptoms, including vocal fatigue, and musculoskeletal pain were observed after the interventions, along with increased MPT in all groups. In the TENS group, breathiness decreased; in the LMT group, there was a significant reduction in overall voice alteration; and in the TENS+LMT group, reductions were observed in overall voice alteration and vocal tension. The LPS also showed a significant decrease in perceived muscle tension during phonation in the TENS+LMT group. Conclusion All interventions showed positive effects, with significant reductions in vocal symptoms and musculoskeletal pain, as well as increases in MPT. The combined techniques were able to provide better vocal adjustments and reduce perceived muscle tension on palpation, demonstrating that both approaches were effective in promoting vocal and musculoskeletal benefits.

The accuracy of a smartphone-based hearing assessment compared to conventional audiometry in workers exposed to noise according to sociodemographic, occupational and pre-assessment conditions Lima, Claire Márcia Santana Fernandes, Luma Cordeiro Rodrigues Corona, Ana Paula

ABSTRACT Purpose To investigate the sociodemographic and occupational factors, as well as the pre-assessment conditions that influence the accuracy of the hearTest in workers exposed to noise. Methods Secondary data from an accuracy study conducted with 232 workers were used. A sociodemographic and occupational questionnaire was administered, and an air conduction hearing assessment of 0.5 to 8 kHz was performed using the hearTest and CA. A Youden index (J) ≥ 70% was considered to define good hearTest accuracy. Results When analyzing the identification of any HL by the hearTest, it was observed that sociodemographic factors (age between 40 and 49 years; white race, low education level, and income above five minimum wages) reduced the accuracy of the assessment. When analyzing the identification of NIHL, sociodemographic factors (age between 40 and 49 years; low level of education; and income above two minimum wages) also reduced the accuracy of the assessment, along with the worker's pre-assessment condition (irregular sleep the night before the assessment). For both HL criteria, it was found that the hearTest presented worse accuracy with increasing age. Workers with higher educational levels achieved greater accuracy, and the J was higher among women, workers in technical roles, those aged between 18 and 29 years, non-white, those with higher education, those with an income of two to five minimum wages, and those who denied fasting, for both HL classifications. Conclusion The findings indicate that sociodemographic, occupational, and pre-assessment conditions can impact the accuracy of the hearTest assessment in workers.

ABSTRACT Purpose To evaluate the immediate effects of low-power laser photobiomodulation followed by the voiced tongue trill technique (VTTT) in women with behavioral dysphonia. Methods This is an experimental study with 30 dysphonic women divided into two groups: Group 1 (G1): placebo LASER followed by VTTT; and Group 2 (G2): infrared LASER at 9 Joules (J) per point (total 63 J) followed by VTTT. Multidimensional assessment included auditory-perceptual evaluation, aerodynamic measurement of maximum phonation time (MPT), acoustic analysis with short-term measurements, cepstral measurements, and extraction of multiparametric indexes, evaluation of self-perceived phonatory effort, and visual-perceptual evaluation of laryngeal images. Results Vocal quality did not differ significantly between the intervention moments or between groups in sustained vowel, but there were positive changes in the overall degree of vocal deviation in the continuous speech task in the placebo LASER group followed by VTTT (p=0.047). G1 had a decrease in jitter (0.025) and pitch perturbation quotient (0.033), and G2 had an increase in MPT (p-value=0.013) and vowel CPPS (p-value=0.027), a decrease in self-perceived phonatory effort (p-value=0.014), and an improvement in vocal fold vibration parameters (p-value=0.017). Conclusion The results show that infrared LASER associated with VTTT improves MPT, voice harmonic structure, self-perceived phonatory effort, and vocal fold vibration pattern of dysphonic women, suggesting that this resource has potential effects on the voice.

RESUMO Objetivo Definir os valores de corte das medidas acústicas extraídas da tarefa de vogal sustentada [a] para discriminação entre presença e ausência de distúrbio vocal em falantes do português brasileiro. Método A amostra foi composta por 376 participantes, 288 do gênero feminino e 88 do gênero masculino, com idade média de 41,20 ± 14,04 anos. Os indivíduos foram submetidos ao exame de avaliação laringológica, e a uma gravação vocal, sendo coletada uma amostra de fala com a vogal [a] sustentada e posteriormente realizado o julgamento perceptivo auditivo e acústico da voz, dos quais 277 foram classificados com distúrbio vocal e 99 sem distúrbio vocal. Foram extraídas 44 medidas acústicas e analisadas por meio da curva ROC para identificação do ponto de corte de cada medida, bem como suas métricas de desempenho. Resultados Foram definidos pontos de corte para 23 das 44 medidas acústicas inicialmente extraídas. Dentre elas, seis medidas demonstraram desempenho robusto e clinicamente relevante para discriminar vozes com e sem distúrbio vocal em falantes do português brasileiro. As medidas com valores de AUC superiores a 0,70 e sensibilidade e especificidade equilibradas acima de 70% foram: Amplitude Variability Index (AVI) (≤0,91), Glottal-to-noise excitation (GNE)2000Hz (>0,92), Spectral Noise Level (SNL)100-2600Hz (≤7,71), SNL100-3000Hz (≤3,38), SNL100-8000Hz (≤10,95) e SNL5100-8000Hz (≤–1,84). Conclusão Seis medidas apresentaram desempenho satisfatório para identificar a presença de distúrbio vocal em falantes do português brasileiro. Dez medidas apresentaram bom desempenho para triagem vocal e sete para confirmação diagnóstica de distúrbio vocal.

ABSTRACT Purpose To define cut-off values for acoustic measures extracted from the sustained vowel [a] task to discriminate between the presence and absence of vocal disorders in Brazilian Portuguese speakers. Methods The sample consisted of 376 participants, 288 female and 88 male, with a mean age of 41.20 ± 14.04 years. All individuals underwent a laryngological evaluation and a voice recording session, during which a sustained vowel [a] sample was collected. Subsequently both auditory-perceptual and acoustic analyses of the voice were performed. Among the participants 277 were classified as having a vocal disorder and 99 as having no vocal disorder. A total of 44 acoustic measures were extracted and analyzed using ROC curves to determine the cut-off value for each measure, along with their performance metrics (sensitivity, specificity and AUC). Results Cut-off values were established for 23 of the 44 extracted acoustic measures. Six measures showed robust and clinically relevant performance in discriminating between voices with and without vocal disorders in Brazilian Portuguese speakers. The measures with AUC values greater than 0.70 and balanced sensitivity and specificity above 70% were: Amplitude Variability Index (AVI) (≤0,91), Glottal-to-noise excitation (GNE)2000Hz (>0,92), Spectral Noise Level (SNL)100-2600Hz (≤7,71), SNL100-3000Hz (≤3,38), SNL100-8000Hz (≤10,95) and SNL5100-8000Hz (≤–1,84). Conclusion Six acoustic measures demonstrated satisfactory performance in identifying the presence of vocal disorders in Brazilian Portuguese speakers. Ten measures showed good performance for voice screening purposes and seven for diagnostic confirmation of vocal disorders.

RESUMO Objetivo Desenvolver um consenso com experts otorrinolaringologistas, fonoaudiólogos e fisioterapeutas acerca dos critérios de recomendação e uso da mindfulness aplicada no tratamento do zumbido. Método Estudo observacional, descritivo, quantitativo e transversal, conduzido de forma online. A amostra por conveniência contou, inicialmente, com 23 especialistas. Aplicou-se o método Delphi em duas etapas, com fornecimento de feedback e anonimato das respostas. A primeira etapa consistiu em perguntas objetivas e subjetivas. A segunda etapa contou com 10 participantes, que avaliaram 34 afirmativas derivadas da etapa inicial. Os especialistas analisaram cada item e demonstraram sua concordância em uma escala likert de cinco pontos. Utilizou-se o coeficiente de validade de conteúdo (CVC) para investigar o grau de concordância entre os juízes e selecionar os itens finais. Resultados Houve consenso em 25 dos 34 itens (CVC ≥ 0,70). Os especialistas consideram que a mindfulness é um recurso terapêutico para o zumbido, porém não como monoterapia. Foi consenso a indicação da aplicação uma vez por semana, pelo menos, 30 minutos, com prática diária domiciliar. O seguimento da terapia após 8 semanas pode ser feito por meio de escalas como Tinnitus Handicap Inventory e a Escala Visual Analógica. Conclusão Há consenso sobre o uso da mindfulness no tratamento (não isolado) do zumbido com protocolo semanal (8 semanas), duração de 30 minutos cada sessão e acompanhamento por meio do THI e EVA. Tais recomendações devem ser exploradas em ensaios clínicos, investigando os efeitos a curto e longo prazo, em prol de evidências científicas e padronização de protocolos clínicos.

ABSTRACT Purpose To develop a consensus among otolaryngologists, speech-language-hearing pathologists, and physical therapists regarding the criteria for recommending and using mindfulness in the treatment of tinnitus. Methods Observational, descriptive, quantitative, cross-sectional study conducted online. The convenience sample initially consisted of 23 specialists. The Delphi method was applied in two stages, providing feedback with anonymous responses. The first stage consisted of objective and subjective questions. The second stage involved 10 participants, who evaluated 34 statements derived from the initial stage. The specialists analyzed each item and demonstrated their agreement on a 5-point Likert scale. The content validity coefficient (CVC) was used to investigate the degree of interrater agreement and select the final items. Results There was consensus on 25 of the 34 items (CVC ≥ 0.70). The specialists consider mindfulness to be a therapeutic resource for tinnitus, but not as monotherapy. There was consensus on recommending application once a week, for at least 30 minutes, with daily home practice. Follow-up therapy after 8 weeks can be done using scales such as the Tinnitus Handicap Inventory and the Visual Analogue Scale. Conclusion There is consensus on the combined (not alone) use of mindfulness in the treatment of tinnitus with a weekly protocol (8 weeks), lasting 30 minutes per session, monitored using the THI and VAS. These recommendations should be explored in clinical trials, investigating the short- and long-term effects to provide scientific evidence and standardize clinical protocols.

RESUMO Objetivo Analisar o desempenho do Montreal Cognitive Assessment Hearing Impairment (MoCA-H) em português brasileiro em idosos neurologicamente saudáveis, comparando aqueles com audição normal e com perda auditiva usuários de próteses auditivas. Método Estudo observacional, transversal e quantitativo, com 20 idosos neurologicamente saudáveis (sem sinais de declínio cognitivo), pareados por idade e escolaridade, divididos em dois grupos: um com média quadritonal dentro da normalidade e outro com perda auditiva bilateral de grau moderado ou superior, usuários de Aparelhos de Amplificação Sonora Individual (AASI). Os participantes foram avaliados por meio de anamnese, audiometria tonal liminar, logoaudiometria, imitanciometria, Mini Exame do Estado Mental (MEEM) e MoCA-H. As comparações entre os grupos foram realizadas com o teste t de Student e Mann Whitney com nível de significância de 5%. Resultados Observou-se desempenho semelhante entre os grupos em sete das oito habilidades avaliadas pelo MoCA-H. A única diferença estatisticamente significativa foi identificada no domínio visuo-espacial/executivo, com desempenho inferior no grupo com perda auditiva. A pontuação total do MoCA-H não apresentou diferença significativa entre os grupos. Conclusão Os idosos neurologicamente saudáveis sem e com perda auditiva tratada apresentaram desempenho semelhante nas tarefas do MoCA-H. Apenas a tarefa visuo-espacial/executivo distinguiu os grupos avaliados. O MoCA-H mostrou-se uma ferramenta eficaz e sensível na avaliação cognitiva de idosos com perda auditiva moderada à severa, ao menos na amostra estudada.

ABSTRACT Purpose To analyze the performance of the Montreal Cognitive Assessment Hearing Impairment (MoCA-H) in Brazilian Portuguese in neurologically healthy older adults, comparing those with normal hearing and those with hearing loss who use hearing aids. Methods Observational, cross-sectional, and quantitative study involving 20 neurologically healthy older adults (with no signs of cognitive decline), matched by age and education, divided into two groups: one with a quadritonal average within normal limits, and the other with bilateral moderate or greater hearing loss, all users of bilateral Hearing Aids (HA). Participants were assessed through anamnesis, pure-tone audiometry, speech audiometry, tympanometry, Mini-Mental State Examination (MMSE), and MoCA-H. Comparisons between groups were conducted using Student’s t-test, with a significance level of 5%. Results Similar performance was observed between groups in seven of the eight cognitive domains assessed by the MoCA-H. The only statistically significant difference was found in the visuo-spatial/executive domain, with lower performance in the group with hearing loss. No significant difference was found in the total MoCA-H score between the groups. Conclusion Neurologically healthy older adults with and without treated hearing loss showed similar performance on the MoCA-H tasks. Only the visuospatial/executive task distinguished the groups evaluated. The MoCA-H proved to be an effective and sensitive tool in the cognitive assessment of older adults with moderate to severe hearing loss, at least in the sample studied.

RESUMO Objetivo Estabelecer pontos de corte e avaliar a acurácia discriminativa dos instrumentos de autopercepção Índice de Severidade da Tosse (CSI-Br) e Questionário Newcastle de Hipersensibilidade Laríngea (LHQ-Br) na identificação de sintomas relacionados à tosse crônica refratária (TCR) em indivíduos adultos. Método Trata-se de um estudo transversal, observacional e quantitativo. A amostra foi composta por 157 participantes, com idade média de 44,92 anos, sendo 111 do sexo feminino e 46 do sexo masculino. Os participantes foram distribuídos em dois grupos a partir da presença ou não de TCR: Grupo com TCR (GT) com 110 participantes, sendo 31 do sexo masculino e 79 do sexo feminino, idade média de 48,67 (DP: 16,04) anos; e, Grupo Controle Saudável (GC) com 47 participantes, sendo 15 do sexo masculino e 32 do feminino, idade média de 36,34 (DP: 14,31) anos. A coleta de dados foi presencial nos ambulatórios dos hospitais participantes e on-line via Google Forms. Os participantes responderam o CSI-Br e o LHQ-Br. A análise foi realizada no software SPSS 29.0, utilizando curvas ROC para determinar pontos de corte e métricas de desempenho. Resultados Para o LHQ-Br, a área sob a curva (AUC) foi de 0,753, com ponto de corte de 5,8, sensibilidade de 70% e especificidade de 73,4%. O CSI-Br apresentou AUC de 0,894 no escore total, com ponto de corte de 4,5, sensibilidade de 84,9% e especificidade de 83%. Conclusão Conclui-se que a partir dos pontos de corte estabelecidos, ambos se mostraram sensíveis e específicos, com maior robustez para o CSI-Br.

ABSTRACT Purpose To establish cutoff points and evaluate the discriminative accuracy of the self-perception instruments Cough Severity Index (CSI-Br) and Newcastle Laryngeal Hypersensitivity Questionnaire (LHQ-Br) in identifying symptoms related to chronic refractory cough (CRC) in adults. Methods This is a cross-sectional, observational, quantitative study with a sample of 157 participants (111 females and 46 males), with a mean age of 44.92 years. They were divided into two groups based on the presence or absence of CRC: Group with CRC (CRC-G) with 110 participants (31 males and 79 females), with a mean age of 48.67 (SD: 16.04) years; The study included a healthy control group (CG) with 47 participants (15 males and 32 females), with a mean age of 36.34 (SD: 14.31) years. Data were collected in person at the outpatient clinics of the participating hospitals and online via Google Forms. Participants answered the CSI-Br and LHQ-Br. Analysis was performed using SPSS 29.0 software, employing ROC curves to determine cutoffs and performance metrics. Results For the LHQ-Br, the area under the curve (AUC) was 0.753, with a cutoff of 5.8, sensitivity of 70%, and specificity of 73.4%. The CSI-Br had an AUC of 0.894 in the total score, with a cutoff of 4.5, sensitivity of 84.9%, and specificity of 83%. Conclusion Based on the established cutoffs, both questionnaires proved to be sensitive and specific, with greater robustness for CSI-Br.

RESUMO Objetivo Mapear e caracterizar as estratégias de intervenção para o condicionamento vocal de cantores sem queixa vocal. Estratégia de pesquisa Revisão de escopo baseada na pergunta: Como se caracterizam as estratégias de intervenção utilizadas para o condicionamento vocal de cantores? Foram realizadas buscas nas seguintes fontes: LILACS, MEDLINE, Embase, Scopus, Cochrane Library, Web of Science, Google Scholar, medRxiv, ProQuest, Journal of Singing e Biblioteca Digital Brasileira de Teses e Dissertações. Critérios de seleção Foram incluídos estudos com treinamento vocal ou respiratório de cantores sem queixas vocais, de qualquer gênero musical, maiores de 18 anos. Excluíram-se estudos com crianças, outros profissionais da voz, participantes com alterações laríngeas e vocais, reabilitação da voz, abordagem exclusivamente indireta ou que não focaram no condicionamento vocal. Análise dos dados Os resultados foram sintetizados em tabelas e quadros. Resultados Foram identificados 12.358 estudos, oito atenderam aos critérios de elegibilidade. Destacaram-se as abordagens com foco em força muscular respiratória e Exercícios de Trato Vocal Semiocluído. O número de repetições juntamente com monitoramento do tempo total de execução foi o mais utilizado. A duração das intervenções variou entre três semanas e seis meses e a maioria dos programas de treinamento foi conduzida por fonoaudiólogos e profissionais de outras áreas. Houve predominância de autoavaliações, com resultados majoritariamente positivos. Conclusão Foi encontrada variabilidade nas estratégias de intervenção utilizadas. O enfoque para um trabalho simultâneo entre respiração e voz se destacou, sendo exercícios de força muscular respiratória e de Trato Vocal Semiocluído as técnicas mais utilizadas.

ABSTRACT Purpose To map and characterize intervention strategies for the vocal conditioning of singers without vocal complaints. Research strategies Scoping review based on the question: How are the intervention strategies used for vocal conditioning of singers characterized? Searches were conducted in the following sources: LILACS, MEDLINE, Embase, Scopus, Cochrane Library, Web of Science, Google Scholar, medRxiv, ProQuest, Journal of Singing, and the Brazilian Digital Library of Theses and Dissertations. Selection criteria Studies with vocal or respiratory training of singers without vocal complaints, of any musical genre, over 18 years of age, were included. Studies with children, other voice professionals, participants with laryngeal and vocal alterations, voice rehabilitation, exclusively indirect approach or those that did not focus on vocal conditioning were excluded. Data analysis The results were summarized in tables and charts. Results A total of 12,358 studies were identified, eight of which met the eligibility criteria. The approaches focused on respiratory muscle strength and Semi-occluded Vocal Tract Exercises stood out. The number of repetitions together with monitoring of the total execution time was the most used. The duration of the interventions ranged from three weeks to six months and most of the training programs were conducted by speech-language pathologists and professionals from other areas. There was a predominance of self-assessments, with mostly positive results. Conclusion Variability was found in the intervention strategies used. The focus on simultaneous work between breathing and voice stood out, with respiratory muscle strength and Semi-occluded Vocal Tract exercises being the most used techniques.

RESUMO Objetivo Mapear e sintetizar as evidências disponíveis sobre o uso da tecnologia Auracast em dispositivos eletrônicos de amplificação sonora (DEAS), no contexto da acessibilidade auditiva em ambientes públicos. Estratégia de pesquisa A revisão seguiu as diretrizes do Joanna Briggs Institute (JBI) e do PRISMA-ScR. Foi utilizada a estrutura PCC (População: indivíduos com perda auditiva usuários de DEAS; Conceito: tecnologia Auracast; Contexto: ambientes públicos). A busca foi realizada em abril de 2025 e abrangeu o período de junho de 2022 a abril de 2025, utilizando o termo “Auracast” como palavra-chave. As fontes de informação incluíram bases de dados científicas (PubMed, Scopus, IEEE Xplore, Dimensions Analytics) e fontes de literatura cinzenta (Google Acadêmico, sites de fabricantes de DEAS, associações profissionais e repositórios institucionais, como o OSF e o OpenGrey). Critérios de seleção A seleção e extração dos dados foram realizadas por dois revisores independentes e cegados, com resolução de conflitos por consenso. Análise dos dados Foram identificados 348 registros. Após a remoção de duplicatas e a aplicação dos critérios de elegibilidade, 17 publicações foram incluídas na síntese final. Resultados As evidências disponíveis apontam para o potencial do Auracast em promover maior acessibilidade e autonomia em espaços coletivos, especialmente quando integrado a DEAS. Entretanto, as publicações encontradas são predominantemente técnicas ou descritivas, com escassez de avaliações clínicas robustas. Conclusão O Auracast representa uma inovação emergente no campo da acessibilidade auditiva, mas ainda são necessários estudos empíricos que avaliem sua eficácia em ambientes educacionais, de saúde e de transporte. A adoção em larga escala dependerá de fatores técnicos, financeiros e sociais, especialmente em contextos com infraestrutura limitada.

ABSTRACT Purpose To map and synthesize the available evidence on the use of Auracast technology in hearing aids (HA), in the context of auditory accessibility in public environments. Research strategies This scoping review followed the Joanna Briggs Institute (JBI) and PRISMA-ScR guidelines. The PCC framework was adopted (Population: individuals with hearing loss using HA; Concept: Auracast technology; Context: public environments). The search strategy was based solely on the keyword “Auracast” and applied to a defined time window from June 2022 (the official launch of Auracast) to April 2025. Information sources included scientific databases (PubMed, Scopus, IEEE Xplore, Dimensions Analytics) and gray literature sources (Google Scholar, websites of HA manufacturers, professional associations, and institutional repositories such as OSF and OpenGrey). Selection criteria Study selection and data extraction were conducted independently and blindly by two reviewers, with disagreements resolved by consensus. Data analysis A total of 348 records were identified. After removing duplicates and applying eligibility criteria, 17 studies were included in the final synthesis. Results The available evidence suggests that Auracast could improve accessibility and autonomy in public settings, particularly when integrated with HA. However, most studies were technical or descriptive, lacking robust clinical evaluations. Conclusion Auracast is an emerging innovation in auditory accessibility. Nevertheless, further empirical studies are needed to assess its effectiveness in educational, healthcare, and transportation settings. Large-scale adoption will depend on technical, financial, and social factors, especially in low-infrastructure contexts.

RESUMO Objetivo Mapear o estado da arte sobre os fatores de risco e métodos avaliativos utilizados no rastreamento dos transtornos da fluência em pré-escolares. Estratégia de pesquisa Revisão de escopo conforme o Instituto Joanna Briggs e checklist PRISMA-ScR. A questão norteadora foi: Quais os fatores de risco e métodos avaliativos utilizados no rastreamento dos transtornos da fluência (gagueira e taquifemia) em pré-escolares na literatura internacional? A estratégia PCC orientou a busca com descritores MESH e termos livres nas bases Lilacs, Pubmed/Medline, Scopus e ScienceDirect, entre 2020 e 2025, sem restrição de idioma. Critérios de seleção Incluíram-se artigos originais disponíveis na íntegra que respondessem a pelo menos uma questão de pesquisa. Excluíram-se publicações não científicas, secundárias, sem resumo, fora do escopo conceitual e populacional, ou com co-ocorrência de outros transtornos. Análise dos dados Os dados foram analisados descritivamente e sintetizados narrativamente. Resultados Identificaram-se 2.091 publicações, após remoção de duplicatas e aplicação dos critérios, 11 artigos foram incluídos. Não foram encontrados estudos sobre fatores de risco para taquifemia. Para a gagueira, mapeou-se 14 fatores de risco mais preditivos relacionados às funções e estruturas do corpo e fatores contextuais. Apenas dois instrumentos padronizados de rastreamento foram identificados, ambos brasileiros: Protocolo de Risco para Gagueira do Desenvolvimento e Instrumento de Rastreio para a Gagueira do Desenvolvimento. Conclusão Estudos sobre fatores de risco para taquifemia em pré-escolares mostram-se ausentes. Para a gagueira, fatores biológicos e contextuais se destacam. A maioria dos métodos avaliativos disponíveis são limitados ao uso do fonoaudiólogo, evidenciando lacunas para pesquisas futuras.

ABSTRACT Purpose To map the state of the art regarding risk factors and assessment methods employed in the screening of fluency disorders in preschool children. Research strategies A scoping review was conducted in accordance with the Joanna Briggs Institute guidelines and the PRISMA-ScR checklist. The guiding question was: What are the risk factors and assessment methods used in the screening of fluency disorders (stuttering and cluttering) in preschoolers, as reported in the international literature? The PCC strategy guided the search, using MeSH descriptors and free terms across the Lilacs, PubMed/Medline, Scopus, and ScienceDirect databases, from 2020 to 2025, with no language restrictions. Selection criteria Original full-text articles addressing at least one research question were included. Exclusion criteria comprised non-scientific or secondary publications, studies without abstracts, those outside the conceptual or population scope, and those involving co-occurring disorders. Data analysis Data were analyzed descriptively and synthesized narratively. Results A total of 2,091 publications were identified; after duplicate removal and eligibility screening, 11 articles were included. No studies on risk factors for cluttering were found. For stuttering, 14 predictive risk factors were mapped, related to body functions and structures as well as contextual factors. Only two standardized screening tools were identified, both developed in Brazil: the Developmental Stuttering Risk Protocol and the Developmental Stuttering Screening Instrument. Conclusion Evidence on risk factors for cluttering in preschoolers is lacking. For stuttering, biological and contextual factors emerged as key predictors. Most available assessment methods are restricted to use by speech-language pathologists, highlighting gaps and opportunities for future research.

RESUMO Objetivo Mapear os estudos que descrevem as práticas de literacia familiar realizadas com crianças pré-escolares. Estratégia de pesquisa Consulta nas bases de dados da Lilacs, EMBASE, PubMed, Scopus, LiVIVO e literatura cinzenta. Registro na plataforma Open Science Framework (OSF). Estratégia de pesquisa “Child, Preschool”, “home literacy", “Early Intervention Educational” e "Family”. Critérios de seleção Foi utilizado o acrônimo PCC: População (Estudos com pré-escolar); Conceito (práticas de literacia familiar); Contexto (ambiente familiar). Análise dos dados Dois autores realizaram a identificação e extração dos dados: autores, ano de publicação, população, objetivo e as práticas de literacia familiar realizadas com os pré-escolares. Resultados Para a síntese final da revisão de escopo foram incluídos 29 estudos. As práticas informais apareceram com mais frequência. Conclusão A prática informal de leitura de livros foi a mais descrita, seguida da leitura compartilhada e o ensino da escrita.

ABSTRACT Purpose To map studies describing family literacy practices carried out with pre-school children. Research strategies Consultation of Lilacs, EMBASE, PubMed, Scopus, LiVIVO and gray literature databases. Registration on the Open Science Framework (OSF) platform. Search strategies: “Child, Preschool”, “home literacy”, “Early Intervention Educational” and “Family”. Conclusion The informal practice of reading books was the most described, followed by shared reading and teaching writing.

RESUMO Objetivo Sintetizar evidências sobre os conhecimentos, habilidades, atitudes, comportamentos e barreiras relatadas por fonoaudiólogos e estudantes de Fonoaudiologia em relação à prática baseada em evidências (PBE). Estratégia de pesquisa Realizou-se uma revisão sistemática com buscas nas bases PubMed/MEDLINE, Scopus, Web of Science, Embase, LILACS, SciELO, LIVIVO e literatura cinzenta. Critérios de seleção Foram incluídos estudos observacionais que investigaram aspectos relacionados à PBE entre profissionais e graduandos da Fonoaudiologia, com dados coletados por meio de questionários. Análise dos dados A extração dos dados foi realizada por dois revisores, de forma independente, e a síntese das informações foi apoiada pela ferramenta de inteligência artificial NotebookLM, com posterior verificação manual. O risco de viés foi avaliado considerando representatividade da amostra, taxa de resposta, precisão dos dados, evidência de cálculo amostral e qualidade do instrumento utilizado. Resultados Foram incluídos 31 estudos publicados entre 2004 e 2024, com amostras variando de 9 a 2.762 participantes. O risco de viés variou de 1 a 4 em uma escala de 0 a 6. Os estudos evidenciam atitudes geralmente favoráveis à PBE, mas indicam limitações importantes nas habilidades de busca em bases de dados, leitura crítica e aplicação das evidências. A prática clínica ainda se baseia fortemente na experiência pessoal, com uso restrito da literatura científica. As principais barreiras relatadas foram falta de tempo, acesso limitado às evidências, escassez de evidências aplicáveis e lacunas na formação. Conclusão Apesar das atitudes positivas, a adoção da PBE na Fonoaudiologia ainda é limitada, exigindo estratégias educacionais e institucionais para fortalecer sua implementação.

ABSTRACT Purpose To synthesize evidence on the knowledge, skills, attitudes, behaviors, and barriers reported by speech-language-hearing pathologists and students regarding evidence-based practice (EBP). Research strategy This systematic review searches the PubMed/MEDLINE, Scopus, Web of Science, Embase, LILACS, SciELO, and LIVIVO databases and grey literature. Selection criteria The review included observational studies that investigated aspects related to EBP among speech-language-hearing professionals and undergraduates, with data collected through questionnaires. Data analysis Two reviewers extracted data independently, and information synthesis was supported by NotebookLM artificial intelligence, with subsequent manual verification. The risk of bias was assessed considering sample representativeness, response rate, data precision, evidence of sample size calculation, and quality of the instrument used. Results 31 studies published between 2004 and 2024 were included, with samples ranging from 9 to 2,762 participants. The risk of bias ranged from 1 to 4 on a scale of 0 to 6. Studies show generally favorable attitudes towards EBP, but indicate important limitations in database search skills, critical reading, and application of evidence. Clinical practice is still heavily based on personal experience, with limited use of scientific literature. The main barriers reported were lack of time, limited access to evidence, scarcity of applicable evidence, and gaps in training. Conclusion Despite positive attitudes, the adoption of EBP in speech-language-hearing pathology is still limited, requiring educational and institutional strategies to strengthen its implementation.
