Linear growth evaluation of asthmatic children

With the aim of studying the impact of atopic asthma on height and growth velocity in children and adolescents, we evaluated 66 patients on the Immunology-Allergy and Pulmonary out patient service, at the State University of Campinas Medical School. The following variables have been analysed for all the patients: gender, age, first wheezing episode, severity, intensity and frequency of asthma, steroids use, bone age and target height. We also studied the monthly family income "per capita", levels of social class and mothers educational background. At the begining of the study, a questionaire was completed; left-hand and wrist x-ray was taken to evaluate the bone age. The patients were divided into two groups according to puberal development, being 40 in the prepuberal group and 26 in the puberal one. Mann-Whitney and Kruskal-Wallis statistical tests were used to analyse height and growth velocity z scores for the variables mentioned above. The Multiple Comparison test was used when the Kruskal-Wallis test resulted significant. The age of prepuberal group varied from 4,3 to 12,2 (7,48 ± 1,889) years, being 26 (65%) male and 14 (35%) female. The time of the observation was 0,5 to 1,3 (0,99 ± 1,722) years and the follow-up time was from 0,0 to 7,52 (1,27 ± 1,53) years. The age of puberal group varied from 7,0 to 14,2 (10,3 ± 1,54) years. The time of observation was 0,6 to 1,5 years (1,04 ± 0,19), being 14 (53,8%) female and 12 (46,2%) male. The height for age z-scores varied from -2,216 to 1,887 (0,028 ± 0,978) and the growth velocity z-scores varied from -2,098 to 2,892 (0,045 ± 1,110) on the prepuberal group. As to the pubere group the z scores were -2,972 to 1,520 (0,170 ± 1,209) for height for age and -5,243 to 6,129 (-0,042 2,321) for growth velocity z-scores. Analysing the z scores as an indicator for height for age in the prepuberal group, two risk factors might be considered for attaining a better height related to the studied group: the patients should be male and also be considered over the target height. Analysing the z-scores as an indicator for growth velocity, two groups were considered risk factors for decreasing the growth velocity: children whose mothers had five-eight years of schooling and children who had recurrent pneumonia. In the puberal group, a high dose of steroids was considered a risk factor for decreasing the growth velocity.
