This experience was made In a general and governamental hospital in São Paulo, from August to September of 1982, as a clinical experience of a discipline "Fundamentals of Nursing II" in Master Graduate Programe, EEUSP. The goals were to learm and implantation of sis-tematized nursnig assistance and orientation of nursing team. The methodology started with a plan that studied four phases: history, assistance plan, prescription and nursing evaluation. It was developed by three students and a teacher, working six hours in morning period, without faults. The units were the medical unit and nefrologic unit and they attended 22 patients. In the evaluation of this work, they suggested the nursing process in three phases: history, prescription and nursing evaluation using the little new instruments.
