Neovascularization of the retina in SC hemoglobinopathy and ^*vitreous hemorrhage

RESUMO

Foram estudados 31 olhos com retinopatia proliferativa falciforme nos estágios III (9 olhos) e IV (22 olhos) de Goldberg em 20 pacientes com genótipo SC. A idade média foi de 31,7a (variação: 19-55; DP: 9,74). O total de 98 lesões proliferativas, ou "Sea Fan" (SF) foram analisadas quanto ao tamanho, número por olho e extensão circunferencial. Foi feita a correlação destas duas últimas variáveis com a presença de hemorragia vítrea (HV). Dos 22 olhos no estágio IV, apenas 7 (31,81%) apresentavam diminuição de acuidade visual relacionada à HV. Dividindo-se o comprometimento circunferencial em intervalos de 15º, concluiu-se que houve correlação entre este e a presença de HV (P=0,0476). A extensão circunferencial de até 30º ocorreu em 9,09% dos olhos no estágio IV e em 55,5% no III, enquanto que lesões mais extensas que 30º ocorreu em 90,91% dos olhos no estágio IV e em 44,45% no III (P=0,0195). Dos 98 SFs, 31,63% tinham 30º ou menos de extensão circunferencial e mais da metade (51,01%) 45º ou menos. Não houve correlação entre o número de SFs e a presença de HV.

Concluimos que olhos com extensão circunferencial de SF maior que 30º têm maior probabilidade de apresentar HV (estágio IV) e que os SFs são, em sua maioria, lesões pequenas de 7 a 8mm de extensão.

SUMMARY

We studied 31 eyes with proliferative sickle retinopathy at Goldberg's stage III (9 eyes) and IV (22 eyes), from 20 SC patients. The mean age was 31.7 years (range: 19-55, SD: 9,74). The size, number per eye and circunferential extention were analysed in 98 sea fan lesions (SF). We did a correlation between the two last variables and the presence of vitreous hemohrrage (VH). When we divided the circunferential extention in 15 degrees intervals, we concluded that the greater the interval, the greater the chance of an eye to present VH (p=0.0476). The circunferential involvement of 30º or less was present in 9,09% of the eyes in the stage IV and in 55.5% of the eyes in stage III, and the sarne involvement larger than 30º occurred in 90.91% in stage IV and in 44.45% in stage III (p=0.0195). From the 98 SFs, 31.63% measured 30º or less and more than a half (51.01%) measured 45º or less. The number of SFs is not related to VH, but we concluded that in eyes with SFs extending circunferentially more than 30º have a greater chance to present with VH (stage IV) and that the SFs are small lesions, usually with 7 or 8mm of extension.
