Fitting contact lenses after penetrating keratoplasty

SUMMARY

The authors revised 40 eyes after (35 patients) fitted with contact lenses after penetrating Keratoplasty, and monitored for an average period of 33.8 months. The major indication for penetrating keratoplasty was keratoconus (65.0%). Post-operative astigmatism in this series ranged from 0.50 to 12.75 dioptries (mean = 4.75 dioptries). Visual acuity of 20/40 or better was achieved in 55% of cases with contact lenses. The bases curves of fitted contact lenses were in the average 2.65 dioptries steeper than the flatest corneal meridian, and lens diameter was in the average 8.7 mm. Thirty-two case (86,5%) were hard lenses (22 gas-permeable and 10 PMMA). Success in fitting contact lenses was achieved in 46%. Contact lenses were not sucessfully fitted in 54%, because of excessive lens movement (8 cases), graft rejection (3 cases) lens intolerance (2 cases).
