Ultrasound biomicroscopy of the anterior segment of eyes with occludable anterior chamber angle

SUMMARY

Purpose:

To study the variations of anterior chambe . (AC) structures in eyes with shallow anterior chamber and occludable AC angle, prior to and after laser iridectomy. The results of dark-room provocative test are reported.

Material and Methods:

Ultrasound biomicroscopy was performed in 25 eyes (17 patients). Central and peripheral scans were performed to permit measurements of AC depth, AC angle, iris thickness and iris-lens contact length, pre and post-YAG laser iridectomy.

Results:

The provocative test decreased AC angle opening (from 11.42 ± 4.8° to 1.75 ± 2.0° (p < 0.05) and central AC depth (from 1.820 ± 0.2 mm to 1.770 ± 0.2 mm (p < 0.05), prior to laser iridectomy. After iridectomy, the provocative test decreased AC angle opening (from 17.93 ± 2.5° to 12.03 ± 2.8° (p < 0.05) and central AC depth (from 1.860 ± 0.2 mm to 1.830 ±0.2 mm (p < 0.05). Iris thickness and iris-lens contact length were considered nonstatistically significant parameters.

Conclusions:

1. The provocative test decreased AC angle opening and central AC depth, prior to and after laser iridectomy. However, after iridectomy there was no angle occlusion; 2. Results were more significant in the eyes with a positive response to the provocative test, justifying their greater susceptibility to acute angle-closure glaucoma.
