Nursing team strategies for caring for older adults in the hospital-home transition: an integrative review

Abstract

Objective

To search the literature for strategies for nursing teams in the transition of care to older adults between the hospital and their home.

Methods

This is an integrative review of the literature with a search in the United States National Library of Medicine (PubMed), Latin American and Caribbean Literature in Health Sciences (LILACS), Cumulative Index to Nursing and Allied Health Literature (CINAHL), Scopus and Web of Science (WoS) databases via the CAPES Journal Portal. Original articles published in English, Spanish and Portuguese were included without delimiting the time of publication. MAXQDA software was used in qualitative data analysis to manage and code the data.

Results

The sample consisted of 14 articles. Two categories of articles were identified that address care strategies performed by nursing in the transition of care between hospital settings and the community.

Conclusion

Transitional care is a fundamental strategy for health promotion, continuity of care, disease prevention and rehabilitation of older adults. Nursing leads in this process.
