Multidisciplinary team approach in amputated patients care: contextualising hospital services and protocols

Abstract

Introduction:

The Brazilian Guidelines for Attention to the Health of the Amputee brings protocols of pre-surgical assistance until post prosthetic fitting rehabilitation.

Objective:

To contextualize pre and post-amputation hospital services and protocols and the process of referral of the amputated person to the rehabilitation and prosthesis, by the SUS, post-hospital discharge.

Method:

Descriptive qualitative research, performed with 23 professionals, among them physicians, physiotherapists, nutritionists, nurses and nursing technicians, through a semi-structured interview. Data collection was performed between June and July 2015 and between April and May 2016.

Results:

The procedures of the pre-surgical phase were related to the consent term, assessment of the limb to be amputated and guidelines. Performing the surgery and choosing the level of amputation depend on the level of vascular injury, trauma or infection. In the postoperative period it was evaluated in the stump: dressing, presence of bone spicules, cicatrization process, ghost pain and mobility. The term curative and bandage was cited as synonymous with stump binding. The guidelines in the hospital setting and for discharge were: care with the stump, compression bandaging with elastic band, positioning and referrals for rehabilitation. Hospital services and protocols follow the Brazilian Guidelines, but behaviors such as assessment and treatment of pain in the stump and ghost, compression bandaging should be encouraged and practiced. Uncertainties reported by professionals were related to referral to both pre and post-rehabilitation process.

Conclusion:

The findings directly affect the amputated person, since the receipt of precise directions facilitates rehabilitation process in a faster and more efficient way.
