The study aimed to describe clients with a diagnosis of heart failure (HF), in that Health Field Model, identifying risk factors. This is a quantitative study carried out through interviews with 37 patients diagnosed with HF using semi-structured questionnaire based on the model of Health Field Model. The mean age of patients was 60 ± 15 years, where most of them was female (59.5%) and presented hypertension (75.7%). The consumption of alcoholic beverages and smoking are carried respectively by 13.5% and 21.6% of patients. The sweets and soft drinks are consumed daily by 21.6% of patients. The diagnosis of HF is unknown (75.7%) and 40.5% of patients seek health services only in emergencies. The determinants of health that make up the Health Field indicated a good environment for the emergence of HF, impairing quality of life for the population.
