Até o momento, não há um método único capaz de diagnosticar com fidedignidade a condição nutricional do paciente com doença renal crônica (DRC). Por essa razão, tem se recomendado o emprego de vários marcadores nutricionais. A avaliação global subjetiva (AGS) se baseia na história física e no exame clínico do paciente e, desde sua criação, novas versões foram elaboradas. A partir da AGS, foi criado o malnutrition inflammation score (MIS), composto por 70% das questões comuns à AGS acrescido de questões objetivas do estado nutricional. Como muitas modificações foram feitas na forma original da AGS e o emprego tanto da AGS quanto do MIS em pacientes com DRC aumentou significativamente na prática clínica, este trabalho tem como objetivo fazer uma revisão sobre a aplicabilidade desses métodos para avaliação do estado nutricional em pacientes com DRC.

Up to now, there is no single method that provides complete and unambiguous assessment of the nutritional status in chronic kidney disease (CKD). Therefore, it has been recommended the use of many nutritional markers. The subjective global assessment (SGA) contains questions regarding the clinical history and physical examination. Subsequently, other versions of the SGA were developed. The malnutrition inflammation score (MIS) was also developed from the original version of the SGA and consists of 70% of the items common to SGA in addition to objective questions. Since many modifications were proposed in the original form of SGA, the use of these questionnaires in CKD patients has increased substantially in clinical practice. Therefore, this paper aims to review the applicability of the SGA and MIS when applied to assess the nutritional status of CKD patients.

Considerando a elevada prevalência de DEP e sua estreita associação com
morbimortalidade, o diagnóstico nutricional nesse grupo merece especial atenção,
principalmente no que se refere ao critério ou método empregado para esse fim. Como

Dentre os marcadores nutricionais subjetivos, os métodos compostos de avaliação do estado nutricional têm ganhado destaque. Estes métodos se baseiam em uma combinação de elementos subjetivos e objetivos do estado nutricional, os quais fornecem um conjunto de informações sobre a condição e o grau de déficit nutricional. No contexto da DRC, a AGS e o malnutrition inflammation score (MIS) se sobressaem como métodos compostos mais empregados tanto na prática clínica quanto em estudos. Por essa razão, este trabalho tem como objetivo fazer uma revisão do uso da AGS e MIS, focando em sua capacidade de diferenciar os pacientes bem nutridos daqueles com DEP, de diagnosticar o grau de DEP (leve, moderada ou grave) e avaliar sua capacidade preditiva de desfechos clínicos.

A AGS compreende um método que engloba aspectos subjetivos e objetivos do
estado nutricional, incluindo componentes da história clínica e do exame
físico. É uma ferramenta simples e de baixo custo, que pode ser aplicada por

A primeira validação da AGS para indivíduos em HD e diálise peritoneal (DP)
se deu em 1993 por Enia et al.1919. Enia G, Sicuso C, Alati G, Zoccali C. Subjective global

1993;8:1094-8. Nesse estudo, verificou-se que pacientes com DEP
diagnosticados pela AGS tinham valores menores de albumina sérica,
percentual de gordura corporal, circunferência muscular do braço e de
ingestão proteica. Posteriormente, um estudo multicêntrico em DP, conduzido
nos EUA e Canadá, o CANUSA (1996), modificou a AGS original e propôs um novo
modelo com uma escala de 7 pontos. Os autores observaram que a escala
expandida fornecia melhor associação com a mortalidade, sendo que o
decréscimo de 1 ponto se associava com um aumento de 25% na

denominado malnutrition inflammation score (MIS). O MIS
conta com um total de 10 componentes, sendo 70% dos itens avaliados comuns à
AGS e os restantes 30% por componentes adicionais (albumina sérica, a
capacidade de ligação do ferro (TIBIC) e índice de massa corporal (IMC)). No
trabalho original do MIS, o aumento da pontuação (mais próxima a 30) se
associou com pior condição nutricional e com maiores taxas de

Capacidade da avaliação global subjetiva e do malnutrition inflammation score em diagnosticar DEP
Um tema importante quando se refere aos métodos compostos de avaliação do estado nutricional é se os mesmos são capazes de diagnosticar corretamente a condição de DEP. Para tanto, deve-se avaliar se esses métodos apresentam boa sensibilidade (verdadeiro positivo) e especificidade (falso positivo) para esse fim. Quando há um método ouro para avaliar o estado nutricional, a sensibilidade e especificidade são avaliadas encontrando o ponto de corte ou valor com melhor capacidade de predizer o desfecho, no caso, a DEP.
Em pacientes com DRC, como não há um padrão ouro para avaliar o estado
nutricional, os guias de conduta na área de nutrição e DRC recomendam que se use
uma combinação de métodos para minimizar os erros com diagnóstico nutricional.
Estes podem ser a antropometria, bioimpedância elétrica, albumina sérica,

trabalhos mostram que pacientes classificados como bem nutridos pela AGS e pelo
MIS apresentaram marcadores nutricionais significantemente maiores do que
daqueles classificados como desnutridos. Os marcadores nutricionais avaliados
nesses trabalhos foram IMC, percentual de gordura, dobras cutâneas,
circunferência da cintura, ângulo de fase, massa celular corporal e albumina
sérica. Com isso, os métodos compostos de avaliação do estado nutricional
mostram-se capazes de diferenciar o paciente bem nutrido daquele com DEP.
Contudo, a capacidade em diagnosticar o grau de DEP (leve, moderado ou grave) da
AGS e MIS não está clara. Trabalhos que realizaram a validação concorrente por
meio da comparação entre os graus de DEP pela AGS com variáveis antropométricas
e laboratoriais mostraram resultados divergentes. Enquanto alguns estudos

A falta de consenso entre os estudos quanto à capacidade da AGS em classificar o
grau de DEP pode ser decorrente da diferença na metodologia dos estudos, tanto
no que se refere ao modelo de AGS empregado, quanto na diversidade de parâmetros
e faixas de normalidade utilizadas para classificar o estado nutricional e fazer
a validação concorrente. Com relação ao MIS, apesar de alguns estudos terem

capacidade do MIS em diferenciar o grau de DEP. Contudo, mais estudos com esse

No que se refere aos modelos de AGS, aquele que apresenta maior precisão para

avaliaram a concordância da DEP
diagnosticada pela massa celular corporal (MCC) (contagem de potássio corporal
total) com o obtido pela AGS em sua forma original, avaliação global subjetiva
patient generated (AGS-PG) e AGS-7p. Entre esses modelos, a
AGS em sua forma original obteve maior concordância com a MCC. Contudo, outros
trabalhos com esse objetivo devem ser realizados para que se defina qual modelo
oferece maior acurácia no diagnóstico de DEP.
A variabilidade intra e interobservador consiste outro ponto importante a se
considerar ao aplicar os métodos compostos. Com esse objetivo, Visser et

receberam treinamento. Em ambos os estudos, notou-se boa concordância
intraobservador e moderada concordância interobservador. Esses achados enfatizam
a importância do treinamento cuidadoso e periódico para aplicação desses métodos
e de se priorizar, sempre que possível, um mesmo avaliador no acompanhamento do
estado nutricional.
Capacidade da avaliação global subjetiva e do malnutrition inflammation score em prever desfecho
A associação entre morbimortalidade e DEP avaliada pela AGS e pelo MIS já foi
descrita em diversos estudos em pacientes com DRC. Estudos incluindo pacientes

que incluiu pacientes em HD acompanhados por até 12 meses para eventos de
mortalidade, merece atenção por mostrar que a chance de morte dos pacientes com
valores de pontuação do MIS acima de 5 era de 80% e acima de 8 de 100%.
Esse conjunto de resultados demonstra, portanto, que a associação entre o pior estado nutricional e aumento de morbimortalidade se faz presente independente do modelo e/ou método composto empregado. Logo, a AGS e o MIS apresentam boa capacidade preditiva de desfechos.
Sumário e conclusão
O emprego de métodos compostos para avaliar o estado nutricional de pacientes com DRC tem ganhado atenção por suas vantagens, tais como a gerar uma avaliação global do estado nutricional utilizando número reduzido de aparelhos para sua realização. Com base nos estudos apresentados, pode-se concluir que esses métodos são capazes de diferenciar o paciente bem nutrido daquele com DEP e que os mesmos apresentam bom poder em predizer piores desfechos de morbimortalidade. Logo, estes métodos constituem uma alternativa válida e de grande aplicabilidade para o diagnóstico de DEP. Contudo, algumas particularidades devem ser consideradas. No que diz respeito à AGS, diferentes modelos foram propostos e não se pode afirmar, ainda, qual modelo oferece maior precisão para o diagnóstico de DEP. Segundo, os pontos de corte propostos para classificar estado nutricional merecem cautela por não representarem, com precisão, o grau de DEP. Nesse sentido, é importante que outros métodos objetivos do estado nutricional sejam empregados para complementar as informações avaliadas pelos métodos compostos. Terceiro, salienta-se a importância do treinamento prévio e cuidadoso dos avaliadores, buscando a redução da variabilidade intra e interobservadores.
