Routes for the construction of a territorial logic of care

This article aims to highlight some contributions in the practice of care of therapeutic accompanying, community health agent and harm reduction work for the construction of a territorial logic of care. These three experiments are linked so closely to the challenges posed by the movement of Psychiatric Reform and Health Reform that are present in the current policies of Mental Health and Primary Health Care. To operate from the concepts of deinstitutionalization and integrality, these practices keep a significant degree of transversality in the context of these policies and are in strategic position for the construction of a model of care that has the territory as the axis articulating the practices.
