This article is a comparative reflection between the so called "filter function" discussed in the paper named "the scientific basis for the gatekeeping role of general practitioners" and the Brazilian experience on primary health care based on a interdisciplinary team. In Brazil there is a devaluation of the clinical function due to an enormous importance attributed to public health. Considerations about enlarged clinic and public health in primary health care are made.
