A study on the characteristics of intravenous drug users who seek treatment in the city of Porto Alegre, Brazil

Introduction: This paper presents data on a sample of inner city injection drug users (IDUs) who sought drug abuse treatment in the city of Porto Alegre, aiming at learning the routes of HIV transmission, generating hypotheses for future studies, and developing guidelines for prevention/intervention programs targeting this population. Methods: Using a structured questionnaire, 142 IDUs were interviewed about their drug use and sexual risk behaviors. Whenever possible, participants were also given referrals for HIV serologic testing. Data on demographic characteristics, sexual behaviors, drug use patterns and HIV serostatus were analyzed, focusing the drug use frequency and drug type and HIV infection rate. Results: Of all participants, 97% reported injecting cocaine (8.6 days per month, 9.3 times per day) and alcohol and marijuana use in the month prior to interview. Of the 44 participants who were tested for HIV, 54.5% were seropositive. Almost 90% of the sample reported having previously received Aids prevention information or risk reduction materials, but behavior changes occurred in only a small proportion of them. In addition, few had received adequate information on proper needle/syringe cleaning. 53% reported having shared injection material, and on average used the same material 16.2 times before discarding it. The sample participants were sexually active, predominantly heterosexual, having sex on average 7 times per month. 44% reported no use of condoms during intercourse, and 48% reported consuming at least alcohol immediately before or during sex. Conclusions: HIV risk behaviors are frequent in the population of inner city injection users, and HIV infection rate is extremely high. Needle cleaning is not a common, maybe because they lack access to information about risky practices. In addition, condom use is rare because they have few partners and frequently use drugs before or during sex, which may interfere with risk reduction. HIV prevention counseling is urgently needed to reduce the risky behaviors of IDUs in Porto Alegre, but it must be tailored to take into account the different needs of both occasional and chronic drug injection users.
