INSTRUCTION TO FILL IN THE INSTRUMENT
This assessment instrument was developed considering structure (resources) and process (activities) indicators that make up the Hearing Conservation Program, based on the existing national law and with a consultation to scientific evidence and documents.
This questionnaire was constructed based on the following documents: Regulatory Norm No. 6 of the Ministry of Labor and Employment (NR 6); Regulatory Norm No. 7 of the Ministry of Labor and Employment (NR 7); Regulatory Norm No. 9 of the Ministry of Labor and Employment (NR 9); Regulatory Norm No. 15 of the Ministry of Labor and Employment (NR 15); Norm of Occupational Hygiene 1 (NHO-1) of FUNDACENTRO; Service Order 608 (OS 608) of the National Social Security Institute; Manual of Guidelines and Minimal Parameters to Develop and Administer the Hearing Conservation Program of FUNDACENTRO.
The instrument aims to estimate the degree of implementation of the HCP - i.e., the condition/level in which the program was/is being implemented.
The instrument is divided into two sets. In the first assessment set, there are 26 questions on the resources necessary to effectively implement the Hearing Conservation Program. These are the basis for the actions that will be implemented. The second set comprises 36 questions on the processes (actions) to be implemented - i.e., the activities that need to be developed in the program.
Each question has a description of the expected answer, setting an assessment parameter. In the assessment of resources, the maximum expected score is 55.0 points. As for the processes, the maximum expected score is 87.0 points. The total expected score for the instrument is 142 points, which would reflect a fully implemented Hearing Conservation Program.
The existence of resources and the implementation of processes following the parameters specified in the instrument must be given the total expected score for each question. The absence or non-presentation of the resources, as well as the non-implementation or partial implementation of processes, are not given any score.
The information can be collected from the administrator of the Hearing Conservation Program or the administrator of the Occupational Health Medical Control Program, depending on the type and form of assessment.
For the analysis of the answers, the scores obtained in each set are summed, and these results are applied in the formula to achieve the degree of implementation of the Hearing Conservation Program, as presented in the sheet for Interpretation and Analysis of the results.
Lastly, the sheet with Results of the Assessment of the Degree of Implementation of the Hearing Conservation Program is filled in, in which the assessor presents the final result of the assessment, the aspects that need improvement, complementary observations, and further instructions aiming at the improvement of the program.

ORIENTAÇÕES PARA O PREENCHIMENTO DO INSTRUMENTO
Este instrumento de avaliação foi elaborado considerando indicadores de estrutura (recursos) e processos (atividades) que constituem o Programa de Conservação Auditiva, baseados na legislação nacional vigente, consulta a documentos e evidências científicas.
Os seguintes documentos subsidiaram a construção deste questionário: Norma Regulamentadora Nº 6 do Ministério do Trabalho e Emprego (NR 6); Norma Regulamentadora Nº 7 do Ministério do Trabalho e Emprego (NR 7); Norma Regulamentadora Nº 9 do Ministério do Trabalho e Emprego (NR 9); Norma Regulamentadora Nº 15 do Ministério do Trabalho e Emprego (NR 15); Norma de Higiene Ocupacional 01 (NHO-1) da FUNDACENTRO; Ordem de Serviço 608 (OS 608) do Instituto Nacional do Seguro Social; Guia de diretrizes e parâmetros mínimos para a elaboração e a gestão do Programa de Conservação Auditiva da FUNDACENTRO.
O instrumento visa à estimativa do Grau de Implantação do PCA, isto é, o estado/nível em que o programa foi/está sendo implantado.
O instrumento é dividido em dois Blocos. No primeiro bloco de avaliação constam 26 perguntas acerca dos recursos necessários para uma eficiente implantação do Programa de Conservação Auditiva, os quais subsidiam as ações a serem implementadas. O segundo bloco é composto por 36 perguntas referentes aos processos (ações) a serem implementados, ou seja, as atividades que precisam ser desenvolvidas no programa.
Cada pergunta apresenta uma descrição do padrão esperado para a resposta, representando um parâmetro de avaliação. Na avaliação dos recursos, a pontuação máxima esperada é de 55,0 pontos. Para avaliação dos processos, a pontuação máxima esperada é de 87,0 pontos. A pontuação total esperada para o instrumento é de 142 pontos, refletindo assim um Programa de Conservação Auditiva totalmente implantado.
A existência de recursos e a implementação de processos de acordo com os parâmetros especificados no instrumento devem receber a pontuação total esperada para cada pergunta. A ausência ou não apresentação dos recursos, bem como a não implementação ou implementação parcial de processos não recebem pontuação.
As informações podem ser coletadas junto ao gestor do Programa de Conservação Auditiva ou gestor do Programa de Controle Médico de Saúde Ocupacional, a depender do tipo e da forma de avaliação.
Para análise das respostas, somam-se as pontuações obtidas em cada bloco e aplicam-se os resultados advindos desta soma na fórmula para obtenção do Grau de Implantação do Programa de Conservação Auditiva, exposta na folha de Interpretação e Análise dos resultados.
Ao final, será preenchida a folha de Resultados da Avaliação do Grau de Implantação do Programa de Conservação Auditiva, onde o avaliador deve apresentar o resultado final da avaliação, os aspectos que precisam ser melhorados, observações complementares e orientações futuras visando a melhoria do programa.
