ABSTRACT
This article aimed to understand the (re)organization of the family care center a result of cardiovascular disease chonic condition. This is a case study guided the approach conceived asin the Calgary Assessment Model in the Family. The unit of analysis was a family experiencesing the cardiovascular disease chronic condition of one of its members. It was that, as the daily routine adapts to the changes generated by the hospitalization a new family configuration of daily living is set up. The implementation of strategiesbthat include the family in hospital discharge planning is suggested.
