We analyzed the accessibility to health services in primary care in the State of Pernambuco and aspects that favor or hinder the entry and permanence of users in the health system. It is a descriptive cross-sectional study based on PMAQ-AB data. There was an expansion of the primary care with geographical accessibility, although units still face some difficulties as for socio-organizational accessibility: architectural barriers to access for disabled and elderly people; the way of receiving people did not impact the reorganization of scheduled actions; regulation centers are insufficient to ensure access; inefficient in the support to diagnoses and procedures/surgeries.
