This is intended to analyze the public-private relations in regionalization of the health system in Espírito Santo. This is a comparative study of two health regions based on secondary data and semi-structured interviews. Two standards of public-private relations were identified - interdependent without conflicts; and multiple arrangements with conflicts - which influenced different dimensions of regionalization. In stands out as conditioning of the patterns observed: system forming trajectory, the service offering structure, medical corporation and philanthropic organizations operation, and the way regionalization is conducted by the State.
