The paper is about the current institutional challenges of the operationalization of the Wheel Method, having as background the Municipal Health System of Sobral, Ceará. It is considered that the Wheels represent the possibility of building collective spaces and of constitution of subjects, with capacity of analysis and intervention as institutional democracy strategy. In its operation, it was evident the predominance of administrative dimension, in detriment of the pedagogical, politics and therapeutics. Workers expressed little political and emotional engagement, which has been held in a demand-execution relationship, with absenteeism of certain professional categories.
