Peribulbar block: an alternative for surgeries of long duration in ophthalmology

SUMMARY

The association of peribulbar anesthetic block (7ml of bupivacaine 0,75% and 3ml of lidocaine 2% and 200 UTR of hyaluronidase or 6ml of bupivacaine 0,75% and 4 ml of lidocaine 2% and 200 UTR of hyaluronidase and 0,1 ml of adrenaline 1:1.000) and sedation with thiopental (2 mg/kg) were evaluated in 512 patients under go ing long duration ophthalmic surgeries. The anesthetic solution Was injected subcutaneously (1,5 ml), into the peribuibar space (3 ml), at both the supero-internal and infero external angles of the orbit. Age, sex, physical status and associated systemic diseases were taking in consideration. Complications related to the method were recorded. The most frequent physical status were ASA II and III, with a high frequency of concurrent diseases. Equimosis and chemoses were observed as intercurrences. Some patients reacted with cephalic movements and sneezing during the infiltration. The low incidence of complications in patients of older age group with concurrent pathologies suggests that the method is an innocuous and efficient option of anesthesia for long duration ophthalmic surgeries.
