Resumo Os condromas extraesqueléticos são pequenas lesões cartilaginosas nodulares que não estão aderidas ao osso ou ao periósteo. São tumores raros que ocorrem comumente nas mãos e nos pés. Objetivo deste artigo é descrever um caso de condroma intramuscular extraesquelético (CIE) no joelho esquerdo e os desafios diagnósticos que enfrentamos. Uma paciente de 25 anos apresentou um edema de crescimento lento no joelho esquerdo havia 2 anos. Clinicamente, o edema era proveniente do músculo quadríceps. Foram consideradas possibilidades como rabdomioma, neurofibroma, e lipoma intramuscular. Os estudos de imagem sugeriram um tumor de gordura benigno. A paciente foi tratada com a excisão cirúrgica. A microscopia indicou CIE sem recorrência. O CIE é uma lesão rara. Clinicamente, pode ter aspecto semelhante ao de outros tumores benignos dos tecidos moles. A histopatologia pode fornecer um diagnóstico definitivo. A excisão cirúrgica do tumor é curativa.

Abstract Extraskeletal chondromas are small nodular cartilaginous lesions not attached to bone or the periosteum. They are rare tumors commonly occurring in the hands and feet. The objective of the present study is to describe a case of extraskeletal intramuscular chondroma (EIC) in the left knee and the diagnostic challenges faced by us. A 25-year-old female patient presented with slow-growing swelling in the left knee for 2 years. Clinically, the swelling was arising from the quadriceps muscle. We considered possibilities such as rhabdomyoma, neurofibroma, and intramuscular lipoma. Imaging studies suggested a benign fatty tumor. She was treated by excision. Microscopy was consistent with EIC without recurrence. A rare entity, clinically, EIC can mimic other benign soft-tissue tumors. Histopathology exams can provide a definitive diagnosis. The excision of the tumor is curative.

Resumo O poroma écrino e o hidradenoma poroide são neoplasias poroides benignas raras, derivadas das glândulas sudoríparas écrinas. Existem quatro tipos de neoplasias poroides, conforme a posição na camada da pele, sendo denominadas hidroacantoma simples, poroma écrino, tumor do ducto dérmico e hidradenoma poroide. As neoplasias poroides geralmente surgem como uma lesão solitária, com crescimento lento, podendo apresentar diferentes apresentações clínicas, como massa sobre o pé, lesão ulcerada, cisto sólido, lesão hemorrágica ou suspeita de melanoma. As extremidades são os sítios mais comuns, especialmente as mãos e os pés. No entanto, a coexistência desses dois tumores em uma única lesão é extremamente rara. A excisão cirúrgica representa o principal tratamento, podendo ser curativa, prevenindo assim as alterações malignas e as recidivas. Descrevemos um raro tumor solitário no pé com características clínicas e histopatológicas de uma associação de poroma écrino e hidradenoma poroide tratado cirurgicamente sem recidiva no acompanhamento de médio prazo. Abstract in English:

Abstract Eccrine poroma and poroid hidradenoma are uncommon benign poroid neoplasms derived from eccrine sweat glands. There are four types of poroid neoplasms according to the position within the skin layer: hidroacanthoma simplex, eccrine poroma, dermal duct tumor, and poroid hidradenoma. Poroid neoplasms usually arise as slow-growing solitary lesions and can present different clinical presentations, such as a foot mass, an ulceration lesion, a solid cyst, a bleeding lesion or suspected melanoma. Extremities are the most common sites, especially hands and feet. However, the coexistence of these two tumors in a single lesion is extremely rare. Surgical excision represents the main treatment and can be curative, preventing malignant changes and recurrence. We describe a rare solitary tumor over the foot with clinical and histopathological features of an association of an eccrine poroma and a poroid hidradenoma that was surgically treated with no recurrence at the midterm follow-up. Irreducible Metacarpal Neck Fracture due to Interposition of the Extensor Tendons Krishna, Sathya Vamsi Kumbar, Rajendra Kura, Sagar Vishwanath De, Soumen Das

Resumo Um jovem do sexo masculino com 15 anos de idade apresentou múltiplas fraturas da mão direita, incluindo fratura do colo do quinto metacarpo com grande angulação. As múltiplas tentativas de redução fechada falharam, sendo então realizada a redução aberta. Foi encontrado durante a cirurgia o tendão extensor interposto dentro da fratura, impedindo desta forma a redução fechada. O tendão foi liberado do sítio da fratura, de modo que foi obtida a redução adequada e a fratura foi estabilizada com fios de Kirschner. A fratura foi bem consolidada, com um bom retorno do movimento e da força. A interposição do tendão extensor é um cenário raro associado a fraturas do colo metacárpico, devendo se suspeitar dela quando houver perda completa de contato entre as extremidades da fratura e as várias tentativas de redução fechada falharem.

Abstract A 15-year-old male presented with multiple right-hand fractures, including a severely angulated small finger metacarpal neck fracture. Multiple closed attempts failed, and open reduction was performed. At surgery, the extensor tendon was found to be interposed within the fracture, thereby preventing closed reduction. The tendon was extricated from the fracture site, adequate reduction was obtained, and the fracture was stabilized using K-wires. The fracture united well, with good return of motion and strength. Extensor tendon interposition is a rare scenario associated with metacarpal neck fractures and should be suspected when there is complete loss of contact between the fracture ends and multiple attempts at closed reduction have failed.

Resumo A mão torta ulnar é uma rara afecção de membros superiores, cujo tratamento depende do grau de comprometimento morfológico e funcional, que se correlaciona com a classificação radiográfica de Dobyns, Wood e Bayne. O objetivo deste estudo é relatar um caso de um paciente de 6 anos de idade, do sexo masculino, acompanhado devido a mão torta ulnar do tipo III (aplasia total da ulna). Apesar da dificuldade inicial de manipular objetos e realizar tarefas cotidianas, o tratamento conservador fisioterapêutico propiciou o ganho de força e o desenvolvimento de habilidades funcionais para a vida diária. Concluímos que pacientes com deformidade do tipo III podem ser manejados adequadamente com reabilitação embora necessitem de seguimento ambulatorial até a maturidade esquelética, pois deformidades dinâmicas e novas limitações funcionais podem levar à necessidade de cirurgias corretivas.

Abstract Ulnar club hand is a rare condition of the upper limbs, for which treatment depends on the degree of morphological and functional impairment, correlating with the radiographic classification of Dobyns, Wood, and Bayne. The aim of the present study is to report a case of a 6-year-old male patient, followed up for type III ulnar club hand (total ulnar dysplasia). Despite the initial difficulty of manipulating objects and performing everyday tasks, conservative physical therapy treatment provided strength gain and development of functional skills for daily life. We conclude that patients with type III deformity can be properly managed with rehabilitation although they require outpatient follow-up until skeletal maturity is reached, as dynamic deformities and new functional limitations may lead to need for corrective surgeries.

Resumo Apesar da relação entre artroplastia de quadril e o desenvolvimento de sarcoma ter sido descrita pela primeira vez na literatura há cerca de quarenta anos, esta associação é extremamente rara. No presente relato de caso, descrevemos a associação entre implante ortopédico e sarcoma de partes moles em um homem de 79 anos submetido a artroplastia primária total do quadril (ATQ) por coxartrose há 24 anos. Foram descritas a evolução clínica e os achados radiográficos e histopatológicos da lesão. No intraoperatório da segunda cirurgia de revisão, foi evidenciada soltura dos componentes acetabular e femoral em associação com extensas áreas de necrose e de metalose. Foi realizado desbridamento da região do quadril e da coxa direita e retirada dos implantes. Devido à extensão da lesão e da necrose, não foi possível realizar nova reconstrução articular. O diagnóstico histopatológico de sarcoma pleomórfico indiferenciado de alto grau associado a extensas áreas de metalose foi estabelecido no tecido adjacente ao implante. O paciente evoluiu com metástases pulmonares e faleceu 6 meses após o diagnóstico. Apesar da raridade da associação, os sarcomas devem ser considerados no diagnóstico diferencial das solturas assépticas, particularmente na presença de metalose no tecido peri-implantar. Pelo nosso conhecimento o período de latência de 24 anos entre a ATQ primária e o estabelecimento do diagnóstico de sarcoma é um dos mais longos relatado até o momento.

Abstract Although the relationship between hip arthroplasty and the development of sarcoma was first described in the literature about forty years ago, this association is extremely rare. In the present case report, we describe the association between orthopedic implants and soft tissue sarcoma in a 79-year-old man who underwent primary total hip arthroplasty (THA) for coxarthrosis 24 years ago. In the present case report, we describe the clinical evolution and the radiographic and histopathological findings of the lesion. In the intraoperative period of the second revision surgery, loosening of the acetabular and femoral components in association with extensive areas of necrosis and metallosis was evidenced. We performed debridement of the hip and right thigh region and removed the implants. Due to the extent of the lesion and to necrosis, it was not possible to perform a new joint reconstruction. The histopatho-logical diagnosis of high-grade undifferentiated pleomorphic sarcoma associated with extensive areas of metallosis was confirmed in tissue adjacent to the implant. The patient developed pulmonary metastases and died 6 months after the diagnosis. Despite the rarity of this association, sarcomas should be considered in the differential diagnosis of aseptic loosening, especially in the presence of metallosis in the peri-implant tissue. To our knowledge, the 24-year latency period between primary THA and the establishment of a sarcoma diagnosis is one of the longest reported to date.

Resumo A artrite séptica é geralmente relatada em pacientes idosos com outras condições médicas subjacentes. Artrite séptica por Escherichia coli é uma infecção rara. Descrevemos o caso de um paciente de 70 anos que apresentou uma fístula supurativa, movimentos limitados do membro inferior direito, e um trauma ocorrido aos 12 anos de idade. Durante todo esse tempo, a fístula esteve presente, secretando pus. Uma investigação clínica detalhada revelou uma infecção piogênica presente na epífise femoral seguida de uma taxa elevada de sedimentação. Após a intervenção cirúrgica, E. coli foi isolada das amostras clínicas, e tomou-se a decisão de colocar pérolas de gentamicina dentro da ferida cirúrgica. O paciente foi tratado com antibióticos. Quatro meses após a intervenção, a fístula supurativa foi completamente curada. Mais tarde, o paciente não estava mais interessado em continuar com o plano de tratamento. Como ele se recusou a remover as pérolas da cadeia de gentamicina e a endoprótese de quadrile, ele foi posteriormente encaminhado para a clínica de atenção primária para manejo conservador e acompanhamento. Ele andava mancando usando sapatos ortopédicos, mas sem muletas ou qualquer outro tipo de dipositivo de auxílio para caminhar. Quatro anos após a intervenção cirúrgica, as pérolas da cadeia de gentamicina ainda estão inseridas no osso. A artrite séptica causada por E. coli pode ficar ativa por décadas, secretando pus e se isolando. Diagnóstico rápido, intervenção cirúrgica adequada, e terapia antimicrobiana são essenciais para o tratamento.

Abstract in English:

Abstract Septic arthritis is usually reported in elderly patients with other underlying medical conditions. Septic arthritis by Escherichia coli is a rare infection. We are describing the case of a 70-years old patient who presented with a suppurative fistula, limited movements of the right lower limb, and a trauma that occurred at the age of 12. Throughout this time, the fistula had been present, secreting pus. A detailed clinical investigation revealed a pyogenic infection present in the femoral epiphysis followed by an elevated sedimentation rate. After the surgical intervention, E. coli was isolated from the clinical samples, and the decision to place gentamicin beads within the surgical wound was taken. The patient was treated with antibiotics. Four months after the intervention, the suppurative fistula was completely healed. Later on, the patient was no longer interested anymore in continuing with the treatment plan. As he refused to remove the gentamicin chain beads and the hip endoprosthesis, he was subsequently referred to the primary care clinic for conservative management and follow-up. He walked with a limp wearing orthopedic shoes and not using crutches or any other type of walking-aid. Four years after the surgical intervention, the gentamicin chain beads are still within the bone. Septic arthritis caused by E. coli can remain active for decades, secreting pus and self-isolating. Prompt diagnosis, adequate surgical intervention, and antimicrobial therapy are essential for the treatment.

Resumo Metacondromatose é uma doença genética autossômica rara com penetração incompleta que envolve função anormal do gene PTPN11. A diferenciação entre tumores condrogênicos é um desafio para os ortopedistas. Relatamos um caso de uma menina de 5 anos com metacondromatose, doença que compartilha atributos com osteocondromas e encondromas. Encontramos múltiplas lesões semelhantes a osteocondromas com a característica atípica de guiar seu crescimento em direção à articulação vizinha (epífise) em vez de se afastar dela. Além disso, as lesões semelhantes a encondromas colunares eram claramente visíveis no raio distal direito, no colo uterino femoral proximal e nas cristas ilíacas. A paciente relatou que algum outro tumor tinha desaparecido ou reduzido com o tempo. Este caso foi debatido entre um grupo multidisciplinar de displasia esquelética. Os achados clínicos e radiográficos acima mencionados reforçam o diagnóstico hipotético da metacondromatose. O diagnóstico definitivo da metacondromatose é uma combinação de achados clínicos, radiográficos e histopatológicos. O diagnóstico diferencial entre encondromas, osteocondromas e metacondromatose é vital devido a diferenças na malignização e na história natural. Quando um paciente tem encondromas múltiplos e osteocondromas com regressão de algumas lesões e característica radiográfica atípica das lesões semelhantes ao osteocondroma apontando para a epífise, a metacondromatose, uma doença rara, deve ser considerada. Tratamento cirúrgico é reservado para lesões dolorosas. O risco de malignização é insignificante e conselhos genéticos devem ser dados por se tratar de uma doença autossômica dominante.

Abstract in English:

Abstract Metachondromatosis is a rare autosomal dominant genetic disease with incomplete penetrance that involves abnormal function of the PTPN11 gene. Differentiation between chondrogenic tumors is a challenge for orthopedists. We report a case of a 5 year-old girl with metachondromatosis, a disease that shares attributes with osteochondromas and enchondromas. We found multiple osteochondroma-like lesions with the atypical characteristic of guiding its growth toward the neighboring joint (epyphisis) instead of moving away from it. Furthermore, columnar enchondroma-like lesions were clearly visible in the right distal radius, in the proximal femoral cervix and in the iliac crests. The patient reported that some other tumor had disappeared or downsized with time. This case was debated between a multidisciplinary skeletal dysplasia group. The aforementioned clinical and radiographic findings reinforced the hypothetical diagnosis of metachondromatosis. Definitive diagnosis of metachondromatosisrequiresa combination of clinical, radiographical and histopathological findings. Differential diagnosis between enchondromas, osteochondromas and metachondromatosis is vital due to differences in malignization and natural history. When a patient has multiple enchondromas and osteochondromas with regression of some lesions and atypical radiographical characteristic of the osteochondroma-like lesions pointing toward the epiphysis, metachondromatosis, a rare disease, must be considered. Surgical treatment is reserved for painful lesions Risk of malignization is insignificant and genetic advice must be given due it is an autosomal dominant disease.

Resumo Sarcoma sinovial é uma rara malignidade de origem mesenquimal; a abordagem diagnóstica geralmente começa documentando um tumor de tecido mole; no entanto, resulta em um diagnóstico desafiador quando é mais profundo, de pequeno porte ou primário do osso. O presente relato descreve um paciente que apresentou dor insidiosa no quadril atribuída a artrite reumatoide, com desfecho fatal devido a doenças de base e complicações cirúrgicas. A subestimação da dor no quadril, principalmente quando não há massa palpável, pode atrasar o diagnóstico.

Abstract Synovial sarcoma is a rare malignity of mesenchymal origin; the diagnostic approach usually begins by documenting a soft tissue tumor; however, it results in a challenging diagnosis when it is more profound, of small size, or primary from the bone. The present report describes a patient who presented insidious onset hip pain attributed to rheumatoid arthritis, with a fatal outcome due to baseline disease and surgery complications. The underestimation of hip pain, mainly when there is no palpable mass, may delay the diagnosis.

Closed-reduction Intramedullary Screw in Fractures of the Proximal Phalanges of the Digits of the Hand: A Series of Three Cases Triwidodo, Arsanto Priambodo, Hendar Nugrahadi Gani, Yogi Ismail

Resumo As lesões esqueléticas mais frequentes são as fraturas que ocorrem nas mãos, que representam aproximadamente 20% de todas as fraturas. As mais comuns são as fraturas falangeanas, que abrangem 6% de todas as fraturas. As fraturas da falange proximal aparecem com mais frequência. O principal objetivo do atendimento é corrigir a anatomia, reduzir os danos aos tecidos moles, e mobilizar os dedos atingidos assim que a estabilização da fratura permitir. Este relato tem como objetivo examinar os efeitos clínicos e da radiação nas fraturas da falange proximal, em pacientes infectados com a fixação de parafuso intramedular que foram submetidos a redução fechada. Este é o relato de três casos consecutivos de fratura da falange: duas fraturas da base e uma fratura simples da diáfise. Todos foram submetidos a redução fechada com parafuso intramedular de compressão sem cabeça. A magnitude pré-operatória da angulação da falange proximal foi em média 30,3° (variação de 13° a 42°). Dois pacientes apresentaram deformidade rotacional. Os achados clínicos foram medidos com a versão abreviada do questionário de Disfunções do Braço, Ombro e Mão (Quick Disabilities of the Arm, Shoulder and Hand, QuickDash, em inglês), em que foram avaliados a amplitude de movimento e os resultados funcionais. Complicações foram observadas em um período mínimo de 3 meses. Os pacientes apresentavam flexo-extensão ativa total na articulação interfalangiana proximal e na articulação metacarpofalangiana de flexo-extensão sem deformidade rotacional. A pontuação no Quick-DASH foi satisfatória (média: 2,3; variação: 0 a 4,5). Não houve cirurgias secundárias, e não se observaram complicações. A fixação intramedular das fraturas da falange proximal com o parafuso de tensão canulado tem se mostrado uma cirurgia segura, com excelente desempenho funcional e recuperação precoce. A fratura é minimizada e consolidada adequadamente pelos pacientes.

Abstract The most frequent skeletal injuries are hand fractures, which constitute around 20% of all fractures. Fractures of the phalanx are usual, comprising 6% of all fractures. Proximal phalanx fractures arise more often. The main goals of the care are to repair the anatomy, reduce the damage to soft tissue, and mobilize the damaged fingers as soon as stabilization of the fracture allows it. The present report is intended to examine the clinical and radiation effects of proximal phalanx fractures in patients treated with intramedullary screw fixation who underwent closed reduction. We report three consecutive cases of proximal phalanx fracture: two basal fractures and one simple shaft fracture. They were treated surgically with closed reduction with intramedullary headless compression screws. The preoperative magnitude of the angulation of the proximal phalanx averaged 30.3° (range: 13° to 42°). Two patients presented rotational deformity. The clinical findings were measured using the abbreviated version of the Disabilities of the Arm, Shoulder and Hand (Quick-DASH) questionnaire, and the range of motion and functional results were assessed. Complications were observed over a span of at least 3 months. The patients displayed complete active flexion-extension proximal interphalangeal joint and flexion-extension metacarpophalangeal joint without rotative deformity. The scores on the QuickDASH were satisfactory (mean: 2.3; range: 0 to 4.5). No secondary surgeries were performed, and complications were not observed. Intramedullary fixation of proximal phalanx fractures with cannulated tension screws has been shown to be a safe operation, resulting in outstanding functional performance and an early recovery. The fracture is minimized and properly consolidated by the patients.

An Unusual Complication of Intramedullary Pin Migration during Total Knee Arthroplasty – A Case Report Londhe, Sanjay Bhalchandra Shah, Ravi Vinod Banka, Paras

Resumo A artroplastia total do joelho (ATJ) é uma das cirurgias mais eficazes para alívio da dor e melhora da função no estágio final da artrose (quando ocorre contato entre os ossos). As várias complicações intraoperatórias da ATJ incluem fratura, lesão em tendão ou ligamentos, e complicações nervosas ou vasculares. Neste artigo, descrevemos uma complicação incomum: a migração do pino intramedular dentro do canal femoral durante a ATJ. Um paciente do sexo masculino de 72 anos foi submetido a ATJ com sistema de estabilização posterior e sacrifício do ligamento cruzado posterior. A porção distal do fêmur foi seccionada, e o equilíbrio foi verificado em extensão. Em seguida, um bloco anteroposterior (AP) cinco em um foi utilizado para seção anterior, posterior, de chanfro, e entalhe. Por apresentar uma saliência medial, o bloco foi deslocado em sentido lateral. Ao fazê-lo, os pinos também tiveram de ser deslocados, e um deles foi inadvertidamente inserido na abertura do canal medular do fêmur criado. Como instrumentos ortopédicos usuais, como pinça reta longa e saca-bocado pituitário não conseguiram remover o pino migrado, uma pinça laparoscópica extralonga foi usada sob controle fluoroscópico para localizar, agarrar e remover o pino migrado.

Abstract in English:

Abstract Total knee arthroplasty (TKA) is one of the most effective operations to relieve pain and improve function in the end stage of osteoarthritis (when bone on bone contact occurs). The intraoperative complications of TKA include fracture, tendon or ligament injury, and nerve or vascular complications. We herein describe an unusual complication of intramedullary pin migration within the femoral canal during TKA. A 72-year-old male patient underwent TKA with a posterior stabilization system with sacrifice of the posterior cruciate ligament. The distal femur was sectioned and balance was checked in extension. Then to make anterior, posterior, chamfer and notch cuts, the five-in-one anteroposterior (AP) cutting block was placed on the distal femur and the cuts were initiated. As there was a medial overhang of the cutting block, it was shifted laterally. While doing so, the pins had to be shifted too, and one of them was inadvertently hammered into the previously-created medullary canal opening of the femur. As usual orthopedic instruments, like the long straight artery forceps and pituitary rongeurs, failed to remove the migrated pin, an extralong laparoscopic grasper was used under fluoroscopy control to locate, grasp, and remove the migrated pin.

Resumo O impacto isquiofemoral (IIF), apesar de infrequente, deve ser pensado como uma das causas da síndrome da dor glútea profunda. A dificuldade em se estabelecer o diagnóstico de IIF e os exames clínicos imprecisos podem ser associados à pequena quantidade de relatos de caso na literatura. O tratamento inicial de IIF utiliza medidas conservadoras, sendo o tratamento cirúrgico pouco frequente. A seguir, está o relato de caso de quatro pacientes adultas com diagnóstico de IIF sem sucesso com tratamento conservador, nas quais realizou-se a ressecção endoscópica do trocanter menor com bons resultados.

Abstract Ischiofemoral impingement (IFI), although infrequent, should be thought of as one of the causes of deep gluteal pain syndrome. Difficulty in establishing a diagnosis and inaccurate clinical examination can be associated with the small number of case reports in the literature. The initial IFI treatment uses conservative measures, and surgical treatment is infrequent. The following is a case report of four adult patients, all female, diagnosed with IFI, with unsuccessful conservative treatments, in whom endoscopic resection of the smaller trochanter was performed with good results.

Resumo Osteomielites fúngicas, principalmente associadas à artrite séptica, são incomuns no Brasil, e por isso às vezes de difícil diagnóstico e tratamento. Relatamos um caso de uma paciente jovem, sem fator de risco imunossupressor, com o desenvolvimento de uma osteomielite e evolução para artrite séptica do quadril. O diagnóstico foi realizado após a sua drenagem cirúrgica e visualização do Cryptococcus Neoformans à anatomia patológica. Foi instaurado o tratamento antifúngico, com remissão total dos sintomas. Como não há consenso em relação ao tratamento de osteomielites fúngicas, descrevemos este relato de caso para atentar ortopedistas sobre a importância do diagnóstico diferencial nas artrites de quadril e a boa evolução do tratamento clínico após a drenagem e isolamento do patógeno.

Abstract Fungal osteomyelitis, especially associated with septic arthritis, is uncommon in Brazil; therefore, sometimes it is difficult to diagnose and treat it. We report the case of a young patient, with no immunosuppressive risk factor, with osteomyelitis leading to septic arthritis of the hip. The diagnosis was performed after surgical drainage and visualization of Cryptococcus neoformans at pathological anatomy. Antifungal treatment resulted in complete remission of the symptoms. Since there is no consensus on the treatment of fungal osteomyelitis, this case report aims to inform orthopedists about the importance of hip arthritis differential diagnosis and the good evolution of clinical treatment after drainage and pathogen isolation.

Resumo A fratura isolada de Tillaux representa uma rara e frequentemente mal diagnosticada fratura de tíbia distal anterolateral em adultos. Normalmente ocorre em adolescentes que se aproximam da maturidade esquelética por avulsão do ligamento tibiofibular anteroinferior. Este estudo de revisão de literatura baseada em caso tem como objetivo lembrar a existência dessa identidade em adultos, resumir seu mecanismo de lesão, diagnóstico e procedimentos de tratamento. De acordo com a literatura, este é apenas o oitavo caso descrito: uma mulher de 46 anos sofreu uma fratura isolada de Tillaux com 4 mm de deslocamento e realizaram-se redução e fixação aberta com parafuso canulado duplo. Após a reabilitação adequada, alcançou-se um excelente desfecho funcional e radiológico. É importante reconhecer e tratar adequadamente essas lesões distintas para evitar maiores instabilidades, alterações degenerativas e limitação da função articular do tornozelo. Um diagnóstico precoce e a osteossíntese apropriada desempenham um papel significativo em um prognóstico de recuperação bem-sucedido.

Abstract Isolated Tillaux fracture is a rare anterolateral distal tibia fracture frequently misdiagnosed in adults. It typically occurs in adolescents nearing skeletal maturity by avulsion of the anterior-inferior tibiofibular ligament. This case-based literature review study aims to elicit the existing information regarding this fracture in adults, and summarize its injury mechanism, diagnosis, and treatment procedures. According to the literature, this is only the eighth case described: a 46-year-old woman that suffered an isolated Tillaux fracture with 4 mm of displacement, and open reduction and fixation with double cannulated screws were performed. After proper rehabilitation, an excellent functional and radiological outcome was reached. It is important to recognize and appropriately treat these distinct injuries to prevent further instability, degenerative changes, and ankle joint function limitation. Early diagnosis and appropriate osteosynthesis play a significant role in a successful recovery prognosis.

Osteoarticular Tuberculosis of the Knee as a Unique Presentation in a 10-month-old Infant: A Rare Case of a Commonly Delayed Diagnosis Gottschalk, Carlos Carvalho, Emanuela da Rocha

Resumo A tuberculose osteoarticular do joelho é uma apresentação incomum da doença, especialmente em crianças com menos de 1 ano de idade. A característica paucibacilar da infecção torna o diagnóstico um desafio, levando em consideração métodos mais clássicos como a cultura e o anatomopatológico. Os fatores de risco são contato com indivíduos com tuberculose bacilífera, estar em uma região de alta prevalência, e populações pediátricas. O presente relato descreve um caso de monoartrite por Mycobacterium tuberculosis, de curso crônico e manifestações inflamatórias intermitentes em um paciente masculino de 10 meses, sem sintomatologia extra-articular e sem história de convívio ou contato prévio com tuberculose bacilífera. A cultura foi negativa e o exame anatomopatológico foi inconclusivo para o agente etiológico e o diagnóstico foi realizado pela detecção de traços de DNA de M. tuberculosis no teste rápido molecular (GeneXpert), utilizando a técnica da reação em cadeia da polimerase. O tratamento foi realizado com medicamentos antituberculose e houve resolução completa do quadro clínico-radiográfico. Este caso enfatiza a importância de considerar a tuberculose como parte dos diagnósticos etiológicos diferenciais iniciais das artrites e, portanto, a necessidade da investigação precoce específica a esta bactéria, ainda que a suspeição clínica não seja elevada.

Abstract Osteoarticular tuberculosis of the knee is an uncommon disease presentation, especially in children under 1 year old. Diagnosis based on classic methods (such as culture and anatomopathological examination) is a challenge due to the paucibacillary characteristic of the infection. Risk factors include contact with individuals with bacilliferous tuberculosis, living in a region with high disease prevalence, and pediatric age group. We describe a case of chronic monoarthritis caused by Mycobacterium tuberculosis and intermittent inflammatory manifestations in a 10-month-old male patient with no extra-articular symptoms and no history of contact with bacilliferous tuberculosis. The culture was negative, and the anatomopathological examination was inconclusive for the etiologic agent. The detection of traces of M. tuberculosis DNA by a rapid molecular test (GeneXpert) based on the polymerase chain reaction technique established the diagnosis. The treatment consisted of antituberculosis drugs and led to complete resolution of the clinical-radiographic picture. This case emphasizes the importance of considering tuberculosis in the initial differential etiologic diagnoses of arthritis and, therefore, the need for an early, specific investigation, even when the clinical suspicion is not high.

Resumo O osteolipoma é uma rara variante benigna do lipoma e constitui menos de 1% de todos os lipomas, se apresentando como uma massa indolor bem circunscrita. É um tumor conhecido por ocorrer em várias regiões, comumente intraósseas ou adjacente ao tecido ósseo, cuja patogênese ainda não está clara. Os exames de imagem são úteis em sua avaliação e, principalmente, no planejamento cirúrgico, que consiste na exérese tumoral. Contudo, o diagnóstico definitivo do osteolipoma é realizado pelo exame histopatológico. Apesar de benigno, o osteolipoma pode comprimir estruturas ao seu redor, levando a importante sintomatologia como neste caso relatado em que está em contato com o plexo braquial.

Abstract Osteolipoma is a rare benign variant of lipoma and constitutes less than 1% of all lipomas, presenting as a well-circumscribed painless mass. It is a tumor known to occur in several regions, usually intraosseous or adjacent to bone tissue, whose pathogenesis is still unclear. Imaging exams are useful in their evaluation and, mainly, in surgical planning, which consists of tumor excision. However, the definitive diagnosis of osteolipoma is made by histopathological examination. Although benign, osteolipomas can compress surrounding structures, leading to important symptomatology, as in this case reported in which it is in contact with the brachial plexus.

Neste trabalho, uma comunicação do tipo Kaplan, porém, ainda não relatada, é descrita a partir de um membro superior esquerdo com uma inusual conformação em alça, ou um looping, entre ramos do nervo ulnar, no osso pisiforme.

Abstract The Kaplan anastomosis is a rare communication originally described between the superficial and dorsal branches of the ulnar nerve, distal to the ulnar tunnel, and in strict relation with the pisiform bone. It reveals, by its particular location, a formation of high clinical-surgical expressiveness. In this paper, we describe a Kaplan-type communication from a left upper limb with an unusual loop conformation between branches of the ulnar nerve in the pisiform bone.

Resumo As fraturas da apófise odontoide são uma das lesões mais frequentes na população idosa, e um problema cada vez mais preponderante com o envelhecimento progressivo da população mundial. Neste trabalho, apresentamos um caso clínico de um doente do gênero masculino, de 88 anos, que sofreu uma queda da qual decorreu uma fratura da apófise odontoide de tipo II de Anderson-D'Alonzo. Dada a idade e suas comorbilidades, optou-se por realizar a osteossíntese da fratura por meio da fixação anterior com parafuso transarticular em combinação com a fixação com parafuso à odontoide. Esta técnica que permite a estabilidade necessária para a consolidação da fratura da apófise odontoide de tipo II de Anderson-D'Alonzo, com a vantagem das menores disseção da musculatura extensora cervical e hemorragia decorrente desta agressão quando comparada com a abordagem posterior, sendo uma técnica à disposição e que acarreta benefícios claros no tratamento desta patologia na população geriátrica.

Abstract Fractures of the odontoid apophysis are one of the most frequent lesions in the elderly population, and an increasingly preponderant problem with the progressive aging of the world population. In the present work, we report a clinical case of an 88-year-old male patient who suffered a fall resulting in a type-II fracture of the odontoid apophysis on the Anderson-D'Alonzo classification. Given the age and comorbidities of the patient, we decided to perform osteosynthesis of the fracture through anterior fixation with a transarticular screw in combination with fixation with an odontoid screw. This technique enables the necessary stability for the consolidation of Anderson-D'Alonzo's type II odontoid apophysis fracture, with the advantage of the lower levels of dissection of the cervical extensor musculature and hemorrhage resulting from this aggression when compared with the posterior approach; moreover, it is a readily-available technique that yields clear benefits in the treatment of this pathology in the geriatric population.

This is a report of a rare case of a female young adult patient with osteonecrosis of the humeral head due to avascular necrosis developed in early childhood. The treatment was tailored and optimized with 3D printing, which helped determine the steps for partial humeral arthroplasty.

Resumo As fraturas acetabulares podem ser acompanhadas por lesões de impacto articular, afetando os desfechos se não vistas ou tratadas de modo incompleto. A impactação marginal detectada em estudos de imagem pré- ou intraoperatórios deve ser reduzida anatomicamente usando a cabeça femoral como modelo e aumentando o defeito com enxerto ósseo, se necessário. A superfície articular do segmento impactado parece voltada para a frente durante a cirurgia, que é a descrição clássica de tais lesões. No relatório atual, descrevemos um padrão incomum de aparecimento de lesões de impacto marginal em dois pacientes, onde a superfície articular do fragmento impactado está voltado para a cavidade articular, sendo o inverso da descrição clássica, aludindo ao mecanismo provável de sua ocorrência, à técnica de redução e às consequências de ignorar tal lesão. Lesões de impacto marginal devem ser diagnosticadas e tratadas corretamente para preservar a congruência articular; no entanto, o cirurgião deve estar ciente da possibilidade de um padrão incomum de impacto marginal no qual o fragmento poderia ser revertido; manter esta possibilidade em mente facilitaria seu diagnóstico e manejo.

Abstract Acetabular fractures could be accompanied by articular impaction injuries, affecting the outcomes if missed or treated less than optimally. Marginal impaction detected either in preoperative or intraoperatively imaging studies should be anatomically reduced using the femoral head as a template and augmenting the defect with bone graft if needed. The impacted segment articular surface looks enface during surgery, which is the classic description of such injuries. In the present report, we describe an unusual pattern of marginal impaction injuries appearance in two patients, in which the impacted fragment articular surface is facing toward the joint cavity, which is the reverse of the classic description, alluding to the probable mechanism of its occurrence, the technique for reduction, and the consequences of missing such injuries. Marginal impaction injuries should be diagnosed and treated correctly to preserve joint congruency; however, the surgeon should be aware of the possibility of an unusual pattern of marginal impaction in which the fragment could be reversed, and keeping this possibility in mind would make its diagnosis and management easier.

Resumo Um schwannoma é uma lesão pouco frequente, sendo um tumor que deriva da bainha de mielina dos nervos periféricos; na maioria dos casos, este tumor é benigno e raramente se apresenta na região do pé e tornozelo. Os pacientes afetados por esse tipo de patologia são geralmente assintomáticos. Ainda assim, às vezes eles têm sintomas neurológicos sensoriais ou motores se o tumor for grande o suficiente para causar a compressão direta ou indireta do nervo afetado. Um paciente do gênero masculino de 55 anos se apresentou ao nosso departamento com inchaço não traumático e dor no aspecto lateral do pé direito e da perna. A ressonância magnética (RM) da perna direita revelou uma lesão bem-circunscrita, medindo 2,5 por 1 cm, mostrando hipointensidade nas sequências T1 e hiperintensidade em T2, compatível com um tumor superficial de células do nervo peronal. Foi realizada excisão cirúrgica da lesão e o exame histopatológico confirmou a suspeita inicial – Schwannoma do nervo peroneal superficial. O pós-operatório foi tranquilo, com melhora progressiva da dor e recuperação funcional completa sem déficits neurológicos. Exame clínico rigoroso associado aos exames de RM permitem diagnóstico adequado, bem como a exclusão de outras patologias com apresentação clínica semelhante. Assim, o cirurgião tem que estar atento a todos os dados para um diagnóstico e tratamento eficazes nesse tipo de patologia rara que não pode ser negligenciada.

Abstract A Schwannoma is an infrequent lesion. This tumor derives from the myelin sheath of the peripheral nerves; in most cases, it is benign and rarely presents in the foot and ankle region. Patients affected by this type of pathology are usually asymptomatic. Still, they sometimes have sensory or motor neurologic symptoms if the tumor is large enough to cause direct or indirect compression of the affected nerve. A 55-year-old male patient presented to our department with non-traumatic swelling and pain in the lateral aspect of the right foot and leg. A magnetic resonance imaging (MRI) scan of the right leg revealed a well-circumscribed lesion, measuring 2,5 by 1 cm, showing hypointensity on T1 sequences and hyperintensity on T2, compatible with a superficial peroneal nerve sheath cells tumor. Surgical excision of the lesion was performed, and the histopathological examination confirmed the initial suspicion—Schwannoma of the superficial peroneal nerve. The postoperative period was uneventful, with progressive improvement of pain and complete functional recovery without neurological deficits. Rigorous clinical examination associated to MRI scans allow adequate diagnosis as well as the exclusion of other pathologies with similar clinical presentation. Thus, the surgeon has to be aware of all the data for an effective diagnosis and treatment in this type of rare pathology that cannot be neglected.

Its symptoms include difficulties in finger and wrist extension with possible radial deviation. Resumo A doença de Legg-Calvé-Perthes (DLCP) origina comumente sequelas na morfologia da articulação coxofemoral. Uma variante comum é a cabeça do fêmur não-esférica e sobredimensionada, associada a um colo femoral curto e grande trocânter elevado, que origina conflito femuroacetabular (CFA). A inovadora técnica de luxação cirúrgica do quadril de Ganz abriu novas possibilidades de tratamento para CFA, incluindo a originada por sequelas de DLCP, sem aumentar o risco de necrose avascular da cabeça femoral. Na coxa magna elipsoide resultante de DLCP, constatou-se que o desgaste articular era mais acentuado na porção central da cabeça do fêmur e que o terço lateral está íntegro, pois não articula com o acetábulo. Para estes casos, foi desenvolvida a técnica de osteotomia de redução da cabeça femoral, que resseca a porção danificada da cabeça femoral e restabelece a sua esfericidade. Os resultados a curto prazo são encorajadores. O presente relato de caso apresenta um paciente com sequelas de DLCP tratado com osteotomia de redução da cabeça femoral.

Abstract Legg-Calvé-Perthes disease (LCPD) commonly causes sequelae in the hip joint morphology. A common variant is an oversized, nonspherical femoral head, associated with a short femoral neck and elevated greater trochanter, which leads to femoroacetabular impingement (FAI). The innovative Ganz technique for surgical hip dislocation opened up new treatment possibilities for FAI, including LCPD sequelae, without increasing the risk of avascular necrosis of the femoral head. In the ellipsoid coxa magna resulting from LCPD, joint wear is more accentuated in the central portion of the femoral head; the lateral third remains intact as it does not articulate with the acetabulum. A femoral head reduction osteotomy technique developed for such cases resects the damaged portion of the femoral head and restores its sphericity. Short-term outcomes are encouraging. The present case report presents a patient with LCPD sequelae submitted to a femoral head reduction osteotomy.

Resumo É apresentado um caso clínico de uma paciente de 41 anos, sem antecedentes pessoais de relevo, que foi estudada por dor abdominal difusa autolimitada, tendo sido detectada incidentalmente uma massa tumoral extrabdominal, extraperitoneal ao nível da chanfradura ciática direita. Durante o estudo do caso, a doente resolveu as queixas abdominais, mas desenvolveu quadro de ciatalgia, irradiada para o pé direito e dor tipo choque elétrico. Biópsia guiada por TAC revelou tratar-se de uma neoplasia mesenquimatosa dos tecidos moles, de baixo grau, com características que a enquadram em um tumor fibroso solitário extrapleural. A paciente foi referenciada para a equipe de bacia do serviço de ortopedia para excisão cirúrgica da lesão, tendo sido operada sem intercorrências – excisão total da lesão, com margens livres, foi obtida e o exame anatomopatológico foi compatível com o realizado na biópsia. Após excisão da lesão, ocorreu resolução completa dos sintomas relacionados com compressão do nervo ciático.

Abstract We present the clinical case of a 41-year-old woman with no relevant personal history. The patient complained of diffuse self-limiting abdominal pain, and we incidentally detected an extra-abdominal, extraperitoneal tumor mass at the level of the right sciatic notch. The abdominal complaints were gone during the initial follow-up, but the patient developed sciatica radiating to the right foot and electric shock-like pain. A computed tomography (CT)-guided biopsy revealed a low-grade mesenchymal neoplasm of the soft tissues with characteristics consistent with a solitary extrapleural fibrous tumor. The pelvis team of the orthopedics department received the patient for surgical excision of the lesion. The procedure occurred with no complications, and we excised the totality of the lesion with tumor-free margins. An anatomopathological examination was compatible with the biopsy assessment. The excision of the lesion resulted in complete resolution of the sciatic nerve compression-related symptoms.

Resumo A síndrome de Gorham-Stout (SGS) é uma doença óssea rara caracterizada pela proliferação anormal de vasos endoteliais e destruição do osso acometido. Por ser comum em crianças e adultos jovens, causa morbidade e mortalidade significativas. Até o momento, não há estratégia terapêutica estabelecida para a SGS. Relatamos um caso raro de SGS no quadril e na crista ilíaca de uma criança.

Abstract Gorham-Stout disease (GSD) is a rare bone disease characterized by an abnormal proliferation of endothelial-lined vessels and destruction of the affected bone. As it affects commonly children and young adults, it is associated with significant morbidity and mortality. To date, there is no established treatment strategy for GSD. We report through this observation a rare case of GSD in a child located in the hip and the iliac crest.

Resumo Neste estudo, apresentamos um paciente do sexo masculino de 4 anos com uma massa indolor de crescimento lento na palma da mão esquerda há 2 anos. Embora os tumores musculoesqueléticos sejam raros, os tumores localizados nas mãos são ainda mais raros em pacientes pediátricos. O fato de pouquíssimos (menos de um em cada dez) tumores serem malignos e existirem dezenas de subtipos, cada um com tratamento diferente, mostra a importância do manejo dessas lesões. O diagnóstico e manejo apropriados de massas de tecidos moles, especialmente tumores malignos insidiosos, são vitais. Devido à raridade dos tumores de tecidos moles, as diretrizes adequadas para seu manejo são limitadas. O objetivo deste relato é apresentar um exemplo de abordagem de um dos tumores de partes moles.

Abstract In this study, we present a 4-year-old male patient with a slowly growing painless mass in the palm of his left hand for 2 years. Although musculoskeletal tumors are rare, hand localized tumors are even rarer in pediatric patients. The fact that very few (less than one in ten) tumors are malignant and there are dozens of subtypes, each with different treatment management, shows the importance of the management of these lesions. Appropriate diagnosis and management of soft tissue masses, especially insidious malignant tumors, is vital. Due to the rarity of soft tissue tumors, adequate guidelines for their management are limited. The purpose of this report is to present an example of the approach to one of the soft tissue tumors.

Resumo Apresentamos o caso de uma paciente de 59 anos com lombalgia crônica causada por tumor intraneural retroperitoneal. A excisão laparoscópica foi realizada e a histologia revelou um neurofibroma da raiz do nervo espinhal. No período pós-operatório, a paciente desenvolveu déficits motores e sensitivos parciais devido ao encarceramento do nervo tumoral, com recuperação progressiva à reabilitação. Este relato revisa a literatura sobre essa doença pouco descrita, destacando a utilidade da laparoscopia em seu tratamento.

Abstract We present a case of a 59-year-old patient with chronic low back pain, caused by a retroperitoneal intraneural tumour. Laparoscopic excision was performed and histology revealed a spinal nerve root neurofibroma. Post-operatively, the patient developed partial motor and sensitive deficits due to tumoral nerve entrapment, with progressive recovery with rehabilitation. This report reviews the literature on this sparsely reported condition, highlighting the utility of laparoscopy in its management.

Resumo O fibroma desmoplásico do osso é um tumor fibrogênico muito incomum, benigno, mas localmente agressivo. Este relato descreve o caso de uma paciente de 45 anos com um enorme fibroma desmoplásico na tíbia proximal. Foi realizado com sucesso um procedimento cirúrgico em dois tempos: ressecção ampla e reconstrução endoprotética. Os cirurgiões devem estar cientes da complexidade de seu tratamento nos casos localmente avançados e agressivos. Uma revisão abrangente da literatura também é fornecida.

Abstract Desmoplastic fibroma of bone is a very uncommon, benign but locally aggressive fibrogenic tumor. This report describes the case of a 45-year-old patient with a massive desmoplastic fibroma of the proximal tibia. A two-staged surgical procedure was successfully performed: wide resection and endoprosthetic reconstruction. Surgeons should be aware of the complexity of its treatment in the locally advanced and aggressive cases. A comprehensive review of the literature is also provided.

Resumo Fraturas atípicas são bem elucidadas quando ocorrem no fêmur e tem relação com o uso de bifosfonatos. A terapia prolongada com esse medicamento leva a uma supressão excessiva do remodelamento ósseo, o que torna o osso mais quebradiço. Em geral, são fraturas atraumáticas ou causadas por trauma mínimo. Este tipo de fratura é também relatada em outros sítios ósseos, como o metatarso. Alguns relatos e estudos referentes às fraturas atípicas de metatarso já foram publicados, mas outras investigações são necessárias para que essa fraturas sejam mais bem compreendidas e recebam o devido diagnóstico, tratamento e conduta. No presente trabalho, relatam-se cinco casos de pacientes que apresentaram fratura de metatarso durante terapia com bifosfonatos. Todos os pacientes eram do sexo feminino, tinham osteoporose como doença pré-existente, faziam uso de bifosfonatos, apresentaram fraturas atraumáticas ou causadas por trauma mínimo, e o exame de imagem revelou fratura metadiafisária transversa de eixo de quinto metatarso com espessamento do córtex lateral, características de imagem semelhantes aos critérios utilizados pela American Society for Bone and Mineral Research (ASMBR) para definir fraturas atípicas de fêmur.

Abstract Atypical fractures are well elucidated when they occur in the femur and are related to the use of bisphosphonates. Prolonged therapy with this drug leads to excessive suppression of bone remodeling, which makes the bone more brittle. In general, they are caused by minimal trauma or are atraumatic. This type of fracture is also reported in other bony sites, such as the metatarsus. Some reports and studies on atypical metatarsal fractures have been published, but further investigations are required to better understand this type of fracture and establish the proper diagnosis, treatment and conduct. The present study is a report of five cases of patients who presented metatarsal fractures during therapy with bisphosphonates. All patients were female, had osteoporosis as a preexisting disease, were taking bisphosphonates, presented fractures that were either atraumatic or caused by minimal trauma, and the imaging examination showed a transverse meta-diaphyseal fracture of the fifth metatarsal shaft with thickening of the lateral cortex, image characteristics similar to the criteria used by the American Society for Bone and Mineral Research (ASMBR) to define atypical femur fractures.

Resumo A patela baixa é uma patologia do joelho pouco frequente, mas limitante devido à rigidez articular e dor localizada na região anterior do joelho. Pode decorrer após trauma, imobilização prolongada ou intervenção cirúrgica local. O achado patológico marcante é o encurtamento e o aumento na espessura do ligamento patelar. Foram descritas várias técnicas cirúrgicas para o seu tratamento, não existindo nenhum tratamento padronizado. Descrevemos o caso de uma doente de 73 anos de idade que apresentava rigidez do joelho, deficit funcional importante e patela baixa após artroplastia total do joelho. Foi submetida a um procedimento cirúrgico de partes moles recentemente descrito, com excelente evolução funcional, melhorando de um Lysholm Knee Score de 16 para 81 pontos, permitindo a mobilização e regresso às atividades de vida diárias precocemente.

Abstract Patella baja is an infrequent knee pathology, but it is limiting due to joint stiffness and localized pain in the anterior region of the knee. It may occur after trauma, prolonged immobilization or local surgical intervention. The striking pathological finding is the shortening and increase in thickness of the patellar ligament. Several surgical techniques have been described for its treatment, and there is no standardized treatment. We describe the case of a 73-year-old female patient who presented with knee stiffness, significant functional deficit, and patella baja after total knee arthroplasty. She underwent a recently described soft tissue surgical procedure, with excellent functional evolution, improving from a Lysholm Knee Score of 16 to 81 points, allowing early mobilization and return to daily life activities.

Resumo Relatamos o caso de uma mulher de 61 anos que deu entrada no pronto-socorro após cair de uma escada. A paciente foi diagnosticada com luxação talar total fechada, sem fratura do tálus ou do tornozelo. O cirurgião indicou uma redução aberta após uma tentativa frustrada de redução fechada. Aos seis meses de acompanhamento, a paciente relata dor branda e descarga parcial de peso sem desconforto; exames de tomografia computadorizada e ressonância magnética não mostraram evidências de necrose avascular ou colapso ósseo.

Abstract We report a case of a 61-year-old female who presented to the emergency room after a fall from stairs. A total closed talar dislocation without talus or ankle fracture was diagnosed. The treating surgeon indicated an open reduction after an unsuccessful attempt at closed reduction. After six months of follow-up, the patient reported mild pain and partial weight-bearing with no discomfort; however, signs of talar avascular necrosis were present on magnetic resonance images and CT scans.
